I love this scene so much. Kate is obviously not letting herself ay her feelings out loud but the looks really say it all. He knows she loves him and she knows he loves her. She’s scared she’ll loose him and all he wants is to protect her. Their chemistry is so good and this is one of the best scenes in television ever. Evie and Josh are both SO GOOD in this scene.
In this episode, Kate's ideal, picket-fence life as a happy bride was all a lie because she was on the run and he didn't know who she really was. Juxtaposed with 'consummating her marriage' with Sawyer, in a smelly bear cage, but it is HONEST and she ain't running .. she's staying put and in this moment she is her true and authentic self, giving herself totally and completely to Sawyer. Later, in "Something Nice Back Home" we will again see her living a lie with Jack, all dressed up in a "picket fence" existence that folds like a pack of cards the minute things get "real". Sawyer is the one Kate belongs with.
They would never have made it in the real world though. Sawyer isn't a guy who's built to work a regular job and make an honest middle class living. Being a slick conman is all he knows how to do (I could also see him as a biker outlaw). And he sure as hell ain't going to college in his 40s. Kate needed someone stable and hardworking to financially provide for her. Otherwise she'd be working extra shifts at Rite Aid.

The actors commentary in this episode and what actually happened in this episode completely contrasted from each other. The way how Kate's flashback and island stories paralleled was that Kate couldn't stay with her husband cause of who she was so she ran but on the island she refused to run and leave Jack behind when she had the chance. She only ran with Sawyer because she had no other choice. Kate sleeping with Sawyer had nothing to do with it. This was nothing but a weak moment of lust. Giving pity sex to someone who's about to die. She knew how Sawyer felt about her and she cared but she didn't care enough. Every time Jack rejects her, she runs into Sawyer arms and vice versa. She clearly doesn't know what she wants. Eventually both men grew tired of her.
