You got this Tochukwu! Most students who fail handwashing include the cleaning of the nailbeds and cuticles in with the 20 seconds. That is a failing mistake. I will be praying you do very well this time around. Take care.

Aloha Dr. Lee, I just want to say how much I appreciate you, I live in Hawaii and passed my state license in 2004. I have since worked in memory care, hospice, rehabilitation, home health and psychiatric units. While working in psych my licence lapsed and now I need to retake exam to move my licence back to active. I was just hired on an acute pediatric psych unit and although I do not need this particular license I want it back. I am also applying for nursing school. Your videos have helped me so much I think over the years skills can turn into something you don’t think about but just do. For instance in home health sometimes you really have to make due with what you have. The tips and tricks you learn along the way are NOT going to help me pass and honestly have made me so nervous. Your videos have given me a crash corse In the steps and I can’t thank you enough. So let this also be some advice to first time test takers your license can and will lapse depending on what department you work on so please be advised to check your state requirements and figure out what you need to do to keep your license or you will have to retake this exam depending on where you are in Hawaii you absolutely will. Best of luck to all and Dr. Lee sending you and your sister so much gratitude for your videos my exam is in two days and I now feel confident I can pass…. Again.
Hello Leanna, Have you checked with the State Nurse Aide Department in Hawaii about renewing your license? I am asking because if you lived in Texas, and have been working with patients (even in psy) the state would renew your certification and NOT have you retest. Sometimes a person will get advice from someone who doesn't know the state requirements. In Texas, a person would: #1 have their employer sign and NOTARIZE a form stating the person has been working in HEALTHCARE for a certain amount of time (it doesn't matter the department, you only have to be working with patients. This includes private duty) #2 submit proof of continuing education hours from an approved agency, and #3 submit a copy of your government ID to state and the person is renewed. For each year that was lapsed, the student must be able to present a notarized statement of working in healthcare from the company or private duty family. I hope this makes sense. Check directly with your state agency, to see if they follow the same guidelines or similar ones like TEXAS. No sense in retesting if you don't have too.

I normally leave the patient's dentures in place unless the nurse ask me to remove them. 2- how to take a rectal temp for an old person, is laying on their stomach? We perfer the patient to be lateral or sims. If the person is prone, you should reposition the patient on their side. who to take care of a prosthetic eye? Not sure what you are asking here. If the patient has an artificial eye, the patient can remove it and clean it as needed. 3- if a wife of patient was complaining about her husband hair not wash daily? do you promise to wash it or let her talk to the charge nurse? You should refer all complaints to the charge nurse.

Now to answer your question, The candidate DOES NOT have to lower the bed or lock the bed wheels during ANY of the skills. Hello LaKisha, Prometric does not give guidance about putting the HOB down after mouth care. I teach my students to leave the HOB elevated at the end of the skill.

Hello, You do not need a barrier on the sink. Please leave the resident sitting up in a fowler's position. A barrier is needed when placing supplies on the overbed table.

Hello, Prometric does not require the student to lock the bed wheels or lower the bed. When you start the skill the bed will be already lowered and locked.

Dr Lee, when people present the skills test, it supposed that the evaluator tell if will be laying down or in a chair? Or the CNA should know? Thank you in advance
Hello, If the skill requires the client to be in a chair, the evaluator will have the client sit in a chair. The evaluator will not tell you what to do. The student needs to know to perform the skill in the chair or bed. I hope this makes sense.
Our students got counted off for this scale. Prometric said "Assist resident position upright 60-90 degrees and offer pillows as needed. Do we have to offer pillows ? and do we have to lay the patient back down to supine or can we leave them in a upright position ?
Hello Kristen, Prometric states for this skill in step #3: Assist resident to sitting position in bed by raising HOB and using pillows as needed to position resident upright at 60° to 90° before offering fluid or brushing teeth? The pillows are only needed if they will help the patient get to a Fowlers position. The pillows are as needed and NOT MANADATORY. Prometric does not give instructions to leave the patient in fowlers or to place the patient in a supine position. Therefore, the evaluator can not grade the student on this issue because there is no step to mark the person on for it. This issue is NOT addressed in the indirect care behaviors handout.

Hello, If the patient is not sitting ALL THE WAY UP in bed, the student should place the patient in High Fowler's position before starting this position.

Hello Nerm, As of today, 8/21/2022, the oral skills are not being tested due to Covid. There is NOT a certain amount of time to brush the teeth. You just need to brush all the surfaces and the Tongue.
Hello, the oral secretions will be poured into the sink. This is similar to a person brushing their teeth at home. The person will put his/her secretions in the sink.

Hello Carolina, You need to do the skill as if you were with a real patient. Would you walk into a client's room and close the curtain, then go out and knock on the door. NO, you would not. Therefore, knock on the door, talk to the client, close the curtain, and then go wash your hand.

Hello, The lowering of the HOB for Prometric is not a requirement for this skill. The candidate MUST raise the HOB (STEP 3) but the state does NOT require that the HOB be lowered. If you see a different rule let me know.

Hello LaKisha, Prometric does not give guidance about putting the HOB down after mouth care. I teach my students to leave the HOB elevated at the end of the skill.

I'm starting again THIS month but im bummed out that its still not face to face because i enjoyed going to the nursing home. In my experience i disliked lab time.

Hello, You do not need a barrier on the sink. Please leave the resident sitting up in a fowler's position. A barrier is needed when placing supplies on the overbed table.

Hello, Prometric does not require the student to lock the bed wheels or lower the bed. When you start the skill the bed will be already lowered and locked.

Dr Lee, when people present the skills test, it supposed that the evaluator tell if will be laying down or in a chair? Or the CNA should know? Thank you in advance
Hello, If the skill requires the client to be in a chair, the evaluator will have the client sit in a chair. The evaluator will not tell you what to do. The student needs to know to perform the skill in the chair or bed. I hope this makes sense.

Hello Sofia, the oral skills have not been added in Texas of as 6-23-22.
Our students got counted off for this scale. Prometric said "Assist resident position upright 60-90 degrees and offer pillows as needed. Do we have to offer pillows ? and do we have to lay the patient back down to supine or can we leave them in a upright position ?

Prometric states for this skill in step #3:
Assist resident to sitting position in bed by raising HOB and using pillows as needed to position resident upright at 60° to 90° before offering fluid or brushing teeth?
The pillows are only needed if they will help the patient get to a Fowlers position. The pillows are as needed and NOT MANADATORY.
Prometric does not give instructions to leave the patient in fowlers or to place the patient in a supine position. Therefore, the evaluator can not grade the student on this issue because there is no step to mark the person on for it. This issue is NOT addressed in the indirect care behaviors handout.

I noticed in some of your videos you give the patient the call light with gloves but in this one you take them off first. Does this matter?
Prometric only requires that the person has the call light before the end of the skill.

Hello Nerm, As of today, 8/21/2022, the oral skills are not being tested due to Covid. There is NOT a certain amount of time to brush the teeth. You just need to brush all the surfaces and the Tongue.

Would you walk into a client's room and close the curtain, then go out and knock on the door. NO, you would not.
Therefore, knock on the door, talk to the client, close the curtain, and then go wash your hand.

Hello, The lowering of the HOB for Prometric is not a requirement for this skill. The candidate MUST raise the HOB (STEP 3) but the state does NOT require that the HOB be lowered. If you see a different rule let me know.
