I agree with the misery part, but I think part of it’s also guilt: Sam living with the knowledge that if he’d killed Jake the first time then Dean wouldn’t be going to Hell. Dean living with the belief (because it’s not true, the demons would’ve found a way anyways) that if he hadn’t gone to get Sam from Stanford, if he’d let him keep living the life he’d wanted, then Sam would be happy with Jessica, enrolled in law school, and not dead in front of him
"I'm a big boy now. I can take care of myself." No hate to Sam but that cracks me up. Time and time again that is proven to be untrue. I do understand his point, it is just funny to me the way he says it.
Sam can be so cruel to Dean sometimes lol Dean does need to worry about himself though. Sam's a big boy and never needed his big brother. Dean needs saving more than Sam sometimes.

It's his character. He can't not worry about Sam. It's what he is. I think it's heartwarming. He's really is the greatest big brother out there.
