My pleliminary application has been verified successfully,and I received a link. I proceed to fill my application form for appointment. On the first page of this document, it is written that ''WNIOSEK O WYDANIE ZEZWOLENIA NA PRACĘ CUDZOZIEMCA NA TERYTORIUM RZECZYPOSPOLITEJ POLSKIEJ''. That document consists of 6 pages. On the document, there is not any sign or stamp, but at the bottom of each page, there is an Identyfikator dokumentu. Is there any place where I can verify this document? Thank you in advance!

Please verify your Travel insurance. The issue may not be with your insurance company, but it may be type and duration of Travel insurance you are Booking. You are asking for a 1 year work visa, so your insurance must Cover from the very day date of your intended departure from your country to the date of your departure from Poland ( 1 year ahead). Now it must be a schenghen visa which covers at least 30 000 euros expenses . Verify you are Booking the right insurance type. And reapply or appeal. Good Luck

On the first page of this document, it is written that ''WNIOSEK O WYDANIE ZEZWOLENIA NA PRACĘ CUDZOZIEMCA NA TERYTORIUM RZECZYPOSPOLITEJ POLSKIEJ''. That document consists of 6 pages. On the document, there is not any sign or stamp, but at the bottom of each page, there is an Identyfikator dokumentu. Is there any place where I can verify this document? Thank you in advance!

Hello madam the number 8 part of the application foam is confusing is it the date of work permit application or passport number in the number of travel document part because its different with what the manual guide is showing

Please verify your Travel insurance. The issue may not be with your insurance company, but it may be type and duration of Travel insurance you are Booking. You are asking for a 1 year work visa, so your insurance must Cover from the very day date of your intended departure from your country to the date of your departure from Poland ( 1 year ahead). Now it must be a schenghen visa which covers at least 30 000 euros expenses . Verify you are Booking the right insurance type. And reapply or appeal. Good Luck
