Improving Care Transitions, Reducing Avoidable Hospital Readmissions

America's health care system faces a dangerous and costly crisis: too many patients are readmitted to the hospital shortly after discharge due to poor care transitions.
The Robert Wood Johnson Foundation is bringing this issue into the national spotlight with Care About Your Care 2013. The initiative focuses attention on improving care transitions to reduce avoidable hospital readmissions. It's also facilitating a national dialogue around how nurses, care coordinators, doctors, consumers, caregivers, patients, and others can work together to achieve this.

There's more than one path to reducing avoidable readmissions. In communities across the country, doctors, nurses, hospitals, care facilities, and community organizations are working with patients to stop the revolving door of avoidable readmissions for good. RWJF asked these front-line team members to share their stories of success in the Transitions to Better Care video contest.
