    Upsc cms consider what harrison and davidson says not what Coaching institute says....
    So tried to complile answers from harrison and davidson..
    If then also u have any doubt or if I m wrong somewhere then do rectify me I'll be glad

      Mam..in 4 th ques..there is systolic murmer .so it may indicative of valvular pathology.so can be the next investigation is echo ?

      Regarding ckd and anemia, Davidson says toxic effects of uremia on BM is a cause for anemia

      Mam in question number 44 .. question gives vesicles in ear ...that means diagnosis should be herpes zoster ..so cranial involvement should be 5 th cranial nerve?

    According to me Answer of 1st question should be A. As in unstable angina treatment is ABE-MOAN theropy and Thrombolysis is contraindicated so best one is A.

      @@healthk3854 standard treatment for nstemi or unstable angina is dual antiplatelets with lmwh , nitrates and an oral beta blocker. so idk wt is th answer

    60th question I have read that Azathioprine don't cause osteoporosis. Lithium causes osteoporosis. Mam please do check it once.

    I think you are right about first one.... option C is not the best management..but given the option it's the right management..never heard of giving ticagrelor ,digoxin or strep. For angina.......anyway thanku ❤️

    Thank you mam. Many questions were as you said, tricky because of the contradictions with coaching class notes. But I think you are right here in keeping strict references to Harrison. Sadly, I got a lot wrong too. You are doing great and it's really helpful, waiting for the rest of it. 👍🏻

    For question 51, Answer Should be 1 and 4 only na? As G6PD is Resistant to malaria, Though I also marked, 1,2,4,? And also one more factor to consider, Primaquine drug treatment of malaria also causes G6PD deficiency?

      It should be 1,4 only, G6pd confers resistance to malaria

      areas where primaquine is used is obviously malaria belt . primaquine causes g6pd. So 6gpd also occurs in malaria belt . Answer is 124

    Ma'am paper looks tough from previous years ? Any expected cutoff for this year for general ?

      This year posts are also more. Last time cut off was 54% i.e 279 marks. This time paper was tougher. So cut off will be at 220 something

      @@145-saiteja4 its like.....if suppose 220/500 is the cut-off. U'll b called for interview. But the job will b assigned only if you clear the final cutoff out out of 600.

    in question 5 its already stated as STEMI then why would we wait for cardiac markers
    As next step we should do echocardiography to know any RWMA EF etc

      Bhai mere It's has mentioned ST elevation in anterior leads, which can have multiplicity of differentials, so Doing Trop in mandatory to confirm it as you are going to give tenecteplase or streptokinase which are quite expensive drugs and also carry their own risks of bleeding. Confirm your diagnosis, then give, anyway 4in1 takes hardly 15 Min

      @@sadbradwithabat2413 door to balloon time is 90min and door to thrombolysis is 30 mins so don't wait for blood tests if we are clinically suspecting STEMI.We have to directly shift the patient to cath lab from there do angiogram along with echo thats the gold standard

      @@sadbradwithabat2413 i have never seen someone diagnosing STEMI using TROP i values Trop i is done to differentiate NSTEMI and Unstable angina
      moreover TROP I might take about 2 to 3 hrs to be positive.even if in NSTEMI with typical symptoms and deteriorating vitals we might directly take the patient to cath lab without waiting for cardiac biomarkers.

      @@hishamkallingal253 the practices might differ but here in our hospital, we first do a 4 in 1 as you can't take everyone to the cath lab, and even in guidelines you first have to confirm it, without it you can't do anything.

      Davidson says toxic effects of uremia on BM can cause anemia in ckd . So it's a controversial qs . We ll be awarded marks .

      It is toxic effect of uremia on rbc and not on bone marrow.

      @@prasanthms318 Its called uremic milieu, which is a term used to denote the toxic effects of uremia on bone marrow, so it is a correct statement, & yes uremia also leads to decrease RBC life span!

      0.33 penalty for 1 wrong answer

      For wrong answer 0.69 is deducted

      How I calculate is +3 for correct answer. -1 for wrong answer. 0 for no response. Finally multiply your score divided by 720 * 500. That will be your score out of 500. For example of 300 out of 720, (300/720)*500 = 208/500 👍🏼

      UPSC doesn't post questions from any coaching class notes or any latest studies unlike NEET PG/INICET.... They put questions directly from standard medical textbooks.
      Hence every question she discussed is probably right....
