  • In this Double Take video from the New England Journal of Medicine, Drs. Nathaniel
Chin and Stephen Salloway review the pathophysiology of Alzheimer’s disease
and discuss how anti-amyloid medications can help in early stages of the
disease. The video notes disparities in diagnosis and treatment of Alzheimer’s
disease and provides guidance on screening and diagnosis and on which
patients should be referred to specialty care. A related podcast, referenced in
the video, is available at www.nejm.org/doi/full/10.1056... : Intention to Treat, episode 20: Alzheimer's
    Update, Part 1 (November 9, 2023).

This is a wonderful addition to the NEJM. Also, I want Dr Chin for my doctor if that time comes for me. What a brilliant and kind clinician!

I would rather have an unkind doctor who gets the job done.

Coconut oil and MCT oil is showing a lot of promise in the energy gap found in early cognitive impairment..definitely worth trying.

Ótimo vídeo de introdução, ficou bem claro sobre a preocupação que os profissionais de saúde devem ter na atenção primária.

Too broad of a discussion. I was left with many questions regarding early symptoms, diagnosis, and age. What are the specific symptoms and at what age should one be concerned. I began forgetting names of close friends and simple words in my early fifties. Everyone, in my age group, brushed it off as age related. Yet, not one of them ever said, “I have the same issues.” I would observe how they, in fact, did not have these issues. My doctors never gave me a cognitive test. They never mention memory loss etc during my annual exam. Ten years later, I my short term memory is terrible. Many times I can’t remember what I did the day before. I can’t place events in time. Did an event occur last week or last month? It’s frustrating as I was once an engineering student who graduated at the top of my class. Today I’m the clueless repetitive grandmother.

I was surprised that hippocampus volume was not mentioned as part of screening. My wife was already in the 3rd percentile vs. age/gender matched controls and did not have any visible plaques via MRI. She was diagnosed with MCI, but with a family history of Alzheimer's, we knew where things were going. Perhaps accordingly, the neurologist pretty much said "we never need to see you again." That was quite discouraging to hear. I like Dr. Chin's bedside manner and approach to patients much better than what we experienced nearly four years ago...

      I have a play-list on dementia reversal.
The people in this video are in the cave man era looking for a new magic profitable drug.
Even in this video they talk about a drug to remove plaques which has been shown to be a failure.

And yes, hippocampal volume is important and is able to be increased.

I am SO GLAD that two of the drugs have been removed or are under review!
We need Medicare for all, AND we need support for healthy diet to help delay onset or slow progression.

Do you have any references to back this up? All my senior female relatives have died with alzheimers/lewy body dementia. The future is terrifying.

I was going to ask similar if you have any references that I could look into myself. I do believe my diet has played into it… I’d be interested to know more if you any specific resources to save the noisy world of advice.
