    OK, got it. I've just enrolled in Yale to get a doctorate in US Medical retirement plans. They've warned me that only 3% of their graduates actually understand all the in's & out's of this and I'd be better off getting a Yale legal degree and joining the lobbyist for healthcare to really take advantage of all of this unnecessary complexity.

    I was listening concentrating on everything in this video suddenly I cracked up insanely because I was thinking these subject matters are for Medicare covered patients and we are older and understanding or keeping track with all these information is truly completely insane. My mind while watching I’m imagining wondering who in the right mind able to have all these confusing coverage being implemented and put in to law for seniors. Remember seniors have , some of us have a short term memory loss, forgetful, can’t think of everything except our medication , to get into the whole game of Medicare is completely terrible to dealt with. Why can’t be seniors treated simple, direct and easier in every level. It just plain painful and worst we are gonna die sooner than every other age group will. appreciate your time and effort to have your valuable videos out there,.

      At a time when we are older and slowing in many ways, this process should be far more simpler. The ands, ifs, or buts, and constant changes are truly a ludicrous expectation for seniors to follow. Surely, this can all be simplified! Your video is excellent but really, and perhaps unwittingly, demonstrates how complicated it all is.

    One thing not mentioned here is that individual pharmacies can charge whatever they want for meds and you need to shop around to see what other pharmacies charge. The plan I'm on encourages CVS as a preferred pharmacy. I was put on 2 new generic cholesterol medications this year. I went to CVS to pick them up and they tried to charge me more than $400 for these 2 generic meds for 90 days each saying that it was because I hadn't yet met my deductible. So, after researching, I transferred both prescriptions to Costco, who charged me $38/$15.99 for the same 90 day supply meds on the same day. I may never meet my deductible, but who cares? CVS has been involved in lawsuits for price gouging. It pays to shop around.

      Yeah, I usually use my Singlcare card. I pay for the lowest priced drug plan-Aetna. I seldom need to use the Aetna drug card. I think I only needed to use it once, as it was actually cheaper for that particular med than using the SingleCare card was.
      I can’t see paying a ridiculous premium for something I don’t really use!
      I really preferred using Walgreens with my employer insurances in the past….but now, my drugs are usually cheaper going to Fry’s (Kroger) Grocery pharmacy with my SingleCare card.

      You know how all of the Medigap supplement plans that are offered by the various insurance companies are exactly the same plan…just that the insurance companies can charge different prices for the same thing? Why aren’t the Part D Drug plans like that?! Why are they so confusing?! If someone did come down with a serious condition, while under one drug plan, how the heck are they supposed to know which drug plan will cover THAT medication the best?!

    This is ridiculous for senior citizens esp those who were not high income earners during their working days and should be exempted from a very convoluted Medicare plan. I buy my meds from coupons or discounts bec it’s cheaper than copays. Whoever is profiting so much from this would definitely answer from his Creator when his time comes.

      The Insurance company's Lobbyist on K-Street. They fund PACs and smooze Congress folks with all kinds of goodies. Perfectly legal by-the-way. It's complicated, research it, and your head will spin.

      If it involves government it will always be complicated. Between government lawyers, company lawyers and who ever else it is that think up these plans it is inevitable for it to confusing. Companies are trying to max out their profits no matter is probably a big part why things are never simple. 😊

    Part D pricing on tier 3,4 and meds was the shocker on the meds currently taking. A&B and supplement premiums was affordable. Unfortunately part D plan premium was also affordable but not the pricing on the formularies on tier 3,4 and 5 . According to two separate brokers the pricing was astronomical out of pocket from 1 through 4 stage . As you stated not many make it through all stages but we will. Luckily we have the financial means to do so. This part ideally needs to change to be more affordable for all.

    This is the first time I've heard that there are more than one type of Part D plan. I pay a lot more than you mentioned. Right now it's $110.20. Yikes!

    I used to not understand why senior get bankrupt eventhough there is so called “Medicare”. Now, I will be one of those seniors that would need medicare soon… This Medicare Part D is very confusing but you explain it very thoroughly. Thanks!

    In Wisconsin (and several other States), they have special State run plans which are approved as 'credible coverage' plans in lieu of Part D. In Wisconsin it is called SeniorCare Rx and is $30 per year. I take very few drugs and they are mostly generics so I use this plan and just pay cash for my drugs (they are inexpensive). Someday, if I need to take expensive meds, I can purchase a Part D plan (without penalty) as the SeniorRx plan qualifies as 'credible coverage'.

    Very informative video. I almost got caught with a over pricing issue. I was recently switching from my former company drug plan to plan D. Seeing the explanation in the video about how the deduction works I ended up calling my insurance agent. After talking with her she contacted the pharmacy I was using and she talked with the pharmacy to see what was causing the over pricing issue. The insurance agent and the pharmacy worked out the issue so that I was able to get a refund. The problem was that the pharmacy had prepared my refill with information from my old company drug plan and not the new information from plan D. The refill sitting at the pharmacy waiting for me to show up to pick it up had a barcode on it that had incorrect information.
    Now any information that is printed up onto a refill has information based on my plan D information.

    Here’s something most don’t know. 1. Medicare doesn’t mandate either a deductible or a % co-pay in the gap phase. However, Medicare does place limits on both and most Part D plans charge the max allowable amt. Max 2024 deductible is $545 & 25% co-pay. Some plans charge 25% in the coverage phase. And, Plans can change the tier, the cost and eliminate a medication at any time during the year. So plan D is only a contract trapping the insured bcs “the plan” is free to change the terms of the contract. The only one held to this “so called contract” is the insured. This is BS & absolutely must change!!!

    I am 69 and can't afford to retire yet. I work for a hospital and have great medical benefits so I only have Part A now. I have epilepsy and have four meds. I got so scared about the possibility of ever having to retire and needing a Part D when I read on the bag of the 90 supply of this one generic med that with insurance I saved $1,580.09!! That generic price alone blows the Eliquis example out of the water. No telling how much my other meds would be. I would hit that donut hole so quickly. Having to pay 25% of the cost of these drugs will be more than I can afford. All I can say is it's been nice knowing everyone, because if I can't afford my seizure meds I will die. It is medically impossible for me to live without my seizures being treated.
    This should be illegal to do this to people on fixed incomes. Hopefully I can keep working forever I guess.

      Some people qualify for the Extra Help program based on income, which significantly lowers their medication costs, and no deductible or coverage gap.

    Thank you for your explanation of a very complex system. The US health insurance and big pharmaceutical companies dominate the system in a corrupt way. The federal government needs to heavily negotiate down the cost of drugs, forbid pharmaceutical benefits management branches of health insurance companies, and not cover very expensive brand drugs at the prices offered by big pharma.

    Amazing video, A friend of mine referred me to a financial adviser sometime ago and we got to talking about investment and money.     This is insane. Very confusing for seniors. This needs to be simplified. The patient becomes the loser since the insurance decides to change price during the year.

      Medicare nerds like me! It's the puzzle my brain loves and craves and I'm really good at teaching Medicare to people (finally a use for that education degree!) I love the demographic and I love helping them make smart choices for their health and their wallet. Hopefully the changes to Part D help those on expensive scripts.     Why is it that NO ONE talks about the plan D late enrollment penalty? If you enroll late you will forever more be saddled with paying the "national" average drug cost+ the 1% penalty for each month you missed + the cost of the private drug plan itself. It is criminal that they add in that "National" average cost because that drives up a $4 - $8 premium by 3x to 4x to like $35 - $50. Absolutely shameful!

      I did not sign up for plan D when I started my medicare because I was not taking any meds. Big mistake. A year or so later, when I needed some meds, I signed up for plan D and have been paying an extra charge every month. Right now it's $25 a month extra. These things are not told to us when we sign up for medicare. Just more money for the government!

      More money to government for sure!!! In April, my husband got disenrolled because he made a mistake not knowing that premium was unpaid, he has severe glaucoma, didn't see the mail, and I was not around to help as I was out of the country. Insurance company didn't care to reinstate his coverage dispite our much energy and effort trying to get the D plan back. He has few expensive meds which we have been paying out of pocket since April. It has been very painful and expensive. I'd called many places and writing long letters but in vain...I'm still feeling very bitter and mad thinking about the ordeal.

    Your videos are, by far, the best source of information on the entire internet. You are a superb spokesman. I do have one question however. At first you said that the $505 deductible doesn't mean that you have to pay the first $505 of drug costs. Later, however, you appeared to be saying exactly that. If I buy a Part D plan and, let's say, have to get a $75 (retail price) prescription filled, will I have to pay the entire $75 initially or just the co-pay? I'm a little confused. Thank you, Sir.       Just the copay until you meet the deductible. If you take low tier or generic drugs, you more than likely won't meet the deductible. He mentioned paying the $505 for someone with an expensive high tier or brand name drug. In that case you will meet that deductible in no time.

    I plan to retire overseas....but keep Medicare going for those times I am back in America (about three months annually). Just in case.
    Prices are cheaper over there for medication and medical care...and I can bring a bit of it back with me for the short duration. I hope I can do this balancing act!

    It is a crime in these United States that senior citizens have to deal with this nightmare called Supplement Plans, and Part D. It is so complicated for seniors; just when a good percentage start loosing their memory.     Regarding the 4 stages of Co-pay. Do the limits, such as the $505 in stage 1, apply to each medication individually resulting in your medications possibly being in different stages or a total of all you medications.

    Regarding your ending comments about not having a Part D when you could have, then starting one up later...Isn't there a permanent penalty you have to pay that goes all the way back to when you started Medicare (or left credible coverage)?

    You said that the drug companies can not take a drug off the list unless the FDA recalls it. But every year I have had lots of times I've called about this and they say they can remove a drug at anytime. I always say that the plan was chosen because of the drugs on their formulary and I agree with what you said about there being a contract but they do not honor that.

    Thank you!! What a mess, why? Because we have taken out the key ingredient, Common Sense. We are all USA citizens, or what we can call Americans. We have put in our time working fir many years to reach this point of retirement. Whether you worked for the government, retired military service, or private industry. Have one plan for all those who have contributed to our system and earned the right to finish out our lives in peace and hopefully enjoyably. Nothing is for free. We all must work, earning the privilege to enjoy our later years of life. This is not a difficult concept. You are born, live, and work your entire life to be able to enjoy our final years in peace without any cost because we paid our dues.     It looks like AARP (United Health Care) has already almost doubled the monthly premiums for 2024, plan D. I just received the notice yesterday. Premium is going from 32 dollars to 68 dollars for the basic plan. Oh, and they changed the name of the plan? So all the hype on the internet about 2024 plans being less that 2023 is out the window at least as far as AARP plan is concerned. Unfortunately there is, as of this date, no other plans to compare. I guess we have to wait for the open enrollment date to see what other providers will be charging in monthly premiums in 2024.

    Is the $4660 max out of pocket per medicine or all medicines combined taken during that year? Also how does the new $35 dollar limit on Insulin work into this equation? I hear Lilly is retailing Insulin for $35 per month. Also wanted to add as FYI, that I went to a prefered pharmacy for a drug and the copay was more than the retail price of the drug at a non preferred pharmacy, somethings not right with these insurance companies and Big pharm.

      All medicines combined. Under a Medicare Part D prescription drug plan, if an insulin is a covered insulin product, the $35 cap for a month’s supply for each insulin product applies, as of January 1, 2023. A covered insulin
      product is one that is included on a Part D sponsor’s formulary. This includes any new insulin products that become available during the plan year. An insulin product might also be considered covered in other instances.

      For my AARP Medicare Advantage Plan 1 (HMO/POS), I was sent a big package when I signed up. Inside that package was a 3 "page" fold-out, titled "Benefit Highlight". On that, it lists the benefits for the following 4 categories:
      Plan Costs, Medical Benefits, Benefits, and Services beyond Original Medicare and Prescription Drugs.
      There is also a list of "Vendor Information" that lists the administrators and contact information for the "perks" of the plan, such as Vision, Dental, Hearing, Nurse Line, Transportation for Doctor Visits, Fitness Memberships, Virtual Medical Visits, and Mental Health Virtual Visits.

    It was a good explanation of how Part D works. I don't really think the video lives up to the somewhat sensationalistic video title - I don't feel that I've learned how to save BIG money and never pay too much. I know you have to attract viewers, but I think there are better ways to word it and still perform well in RUclips's search function.
    The whole system is ridiculously complicated and difficult to understand. Personally, trying to exist on mostly just Social Security, I think a plan with a premium, $540 (2024) initial deductible, PLUS a high co-pay for at least one med (Tier 3 medication) PLUS a fluctuating co-pay (I'm sure it fluctuates up, I bet it doesn't go down) in the donut hole, PLUS a flat 25% co-pay in Catastrophic is horrifically complicated, potentially unaffordable, hard to understand, and somewhat scary - why would I want to sign up and pay money for this? It looks like I'm going to have to drop my Tier 3 drug altogether until a generic comes out. It'll be interesting to see if I can outlive the donut hole.
    In short, tell me you don't really care about senior healthcare without telling me you don't care about senior healthcare! (Not YOU, I'm talking about whoever hammers out Medicare drug policy)
    It's complicated scenarios like this that drive people to Advantage because it's just *easier*. Of course all of that isn't any fault of yours, but it just goes to show how important it is that every element of your videos delivers on the promise. Or the premise.

    If I only take out original Medicare A & B but decide not to enroll in any Part D - can my physician then prescribe any medicine for me that he wants to?
    For instance- original Medicare part d does not approve some of of my medications- such as estrogen for women 65 & older- so I do not want Medicare part d. Having part a & b will not interfere in what descriptions I can get - is that right ? Please help me know this answer

      Not having prescription coverage could be detrimental later on. Not enrolling in a Part D plan will result in lifetime penalties if you attempt to sign up for one when you need it. Even if a prescription you need isn't covered by Medicare or any offered plans, it's still wise to sign up for a low-cost Part D plan to avoid penalties. For prescriptions not covered, you can use services like GoodRx, or ask your doctor about coupons or samples that may be available. This way, you maintain coverage and avoid penalties while managing costs for your specific medications.

      No, unfortunately it does not. Keep in mind, Not all Drug tiers are subject to the deductible. Some plans have Generic Medications on tier 1 & 2, deductible exempt. Also, when you're in the deductible phase, if you have a medication that applies to it, you pay the retail cost of that medicine, until you reach $505 for the year.

      I have an appointment with my GP in October and am requesting 90 day prescriptions for my 2 meds before I go on Medicare Nov 1st. That way I won’t need any meds (hopefully) until next year.

    No one is ‘happy’ in catastrophic. Why? First, they are very sick to get to this phase. The second reason is you need to pay 5% which on an expensive cancer drug that costs $20,000 prr month is $1,000 per month OOP and their is no cap on what you spend.

    I pay $20.00/month for my meds now. July 1, 2023...they will 70.00/month because the part D plan I discussed with (the first guy you hired for) your company doesn't cover one of my medications.
    A, B, D and G. I will find out after April 10th how much my part B is going to cost😬

      GoodRx is a free prescription drug discount program. Medicare does not see that as credible coverage so simply using that and forgoing a Medicare prescription drug plan will eventually cause you to have a penalty if you later decide to enroll into a Medicare prescription drug plan.
