What amazes me is that the letters, Japanese characters and occationally pictures are actually notes that form the music at some points. Also the dedication and effort gone into making this is amazing.
The change from Phantom Ensemble to Ancient Temple at 4:23 is so smooth as to be imperceptible, which makes the Youmu fight portion of the stage theme hit seemingly out of nowhere; this medley is incredible.
The flow between Doll Judgement, Doll Maker of Bucuresti, Romantic Children, and as it flows out into Capital City of Flowers in the Sky starting at say 2:40 is perfect in every way possible, so much so it is hard to listen to the invidiual music without them flowing together like in this arrangement in my head.
I'll never understand how people can put symbols or words into these and make it actually sound appealing to the ears. This is amazing, simple as that.

@GmanX50 Yep! Most people use Innocent Treasures as a basis in medley simply because of its being ZUN's theme. Try searching for the original version of Innocent Treasures, which is from ZUN's CD release "Changeability of Strange Dream." It should help tell you what the tune is supposed to sound like.
