People all around began to look like him because John's brain couldn't accept the fact that Sherlock wasn't alive and next to him basically 24/7. This was his method of coping. Next, we come to the restaurant scene, and in my opinion, the most definitive evidence of this theory. John, with his military background, must've scanned and seen all of the people in the room including Sherlock in his "disguise," and Sherlock goes near the table more than enough times for John to get a good look at him. Here is the scary part. Considering the repetition and irrefutable likeness, John must be consciously suppressing anything including Sherlock. What this demonstrates is that John can't distinguish delusion from reality. Sherlocks appearance in his life occurs so frequently and vividly that anything he thinks he sees he immediately denies, no matter how striking the similarity. The real Sherlock could've stood in front of him, and he wouldn't recognize him.
