That's a good question. Immunosuppressive agents decrease the activity of the immune system. Immunomodulator can increase, decrease, or just correct the working of an altered immune system. That is the simple difference. Suppressives only decrease, while modulators make sure that the immune system works correctly as it should. Hence, mmf is immunosuppressive and immunomodulator, but apremilast is only an immunomodulator. So all immunosuppressives are immunomodulator, but only a subset of immunomodulators are immunosuppressives. I hope it is clear. Let me know if it's not
