    I just had endometriosis removed from my pouch of Douglas. It makes so much sense now as to why I'd get pain upon going from lying down to standing during the second half of my cycle. Great explanation!

    U r amazing, please please please could u do one on endometriosis in tye pouch of douglas and how it would affect not just that area but up the spine and in hips. 13 years with this and at 43 am really in so much pain. Am trying to understand. Thank u 🙏🏽

    When i was scrubbed into a surgery, the surgeon asked me about the pouch of douglas. We had the chance to remove a pelvic abscess.
    One difference between men and women: since men do not have a uterus, they are more likely to form rectovesical fistulas, while women, having that uterus in between, form rectovaginal fistulas.
    Pelvic abscesses sit in the pouch of Douglas from things such as crohn's disease, diverticulitis, etc.

    Great visual explanation. I am biologist , but haven’t know that place in human body 😬 Thank you! My gynecologist noticed 6 months ago 4ml of free liquid in Douglas pouch, now they are 6 ml 🫣 I am under periodic observation and hope this is nothing to worry about 🙏

    Also pouch of Douglas is the most common location of abdominal pregnanacy which is a rare ectopic pregnancy. Not to confuse with the most common ectopic pregnancy location which is ampulla of fallopian tube!

    Just a little note about the first time I ever heard about the pouch of Douglas was in a Netflix video called Douglas by Australian comedian Hannah Gadsby.
    Her dog is named Douglas but she tells the story about a male physician called James Douglas who "discovered" it!

    Only thing I noticed was you said naming was superficial to deep, which is relative to an anterior approach. Should be anterior to posterior instead, which is an absolute description

      Superficial to deep is relative to the surface of the body. E.g. skin is superficial to muscle, bones are deep to muscle.

    I am 27yrs old and I’m just told that I have copious fluid in pouch of Douglas after an ultrasound. I will be returning for second opinion and solution tomorrow but, I am scared. I worry about reproduction and possibly corrective measures. Are they side effects if I go through a surgery?
    What kind is required for correction?

    You mentioned that the pouch of Douglas is the inferior-most space of the peritoneal cavity but wouldn’t it be more correct to say ‘of the abdominal cavity’ as technically the peritoneal cavity is the small cavity between the visceral & parietal layers of the peritoneum and not the two parietal layers (uterine & rectal parietal peritoneum)? I might be overthinking this. Thank you very much in advance!

      You can make this even more confusing: the peritoneum of the pouch of Douglas is covering organs so it is visceral peritoneum. Or is it? Plus, it is in the pelvis, so it is not the abdominal cavity. Or is it? The pouch of Douglas is part of the peritoneal cavity and is continuous with the other parts of the peritoneal cavity. It is difficult to describe and understand the peritoneal cavity, so the description of the peritoneal cavity being between parietal and visceral peritoneum is a good starting point, but that description doesn't hold up everywhere. Tricky!
