I just had endometriosis removed from my pouch of Douglas. It makes so much sense now as to why I'd get pain upon going from lying down to standing during the second half of my cycle. Great explanation!

When i was scrubbed into a surgery, the surgeon asked me about the pouch of douglas. We had the chance to remove a pelvic abscess. One difference between men and women: since men do not have a uterus, they are more likely to form rectovesical fistulas, while women, having that uterus in between, form rectovaginal fistulas. Pelvic abscesses sit in the pouch of Douglas from things such as crohn's disease, diverticulitis, etc.

I worry about reproduction and possibly corrective measures. Are they side effects if I go through a surgery? What kind is required for correction?
You mentioned that the pouch of Douglas is the inferior-most space of the peritoneal cavity but wouldn’t it be more correct to say ‘of the abdominal cavity’ as technically the peritoneal cavity is the small cavity between the visceral & parietal layers of the peritoneum and not the two parietal layers (uterine & rectal parietal peritoneum)? I might be overthinking this. Thank you very much in advance!
You can make this even more confusing: the peritoneum of the pouch of Douglas is covering organs so it is visceral peritoneum. Or is it? Plus, it is in the pelvis, so it is not the abdominal cavity. Or is it? The pouch of Douglas is part of the peritoneal cavity and is continuous with the other parts of the peritoneal cavity. It is difficult to describe and understand the peritoneal cavity, so the description of the peritoneal cavity being between parietal and visceral peritoneum is a good starting point, but that description doesn't hold up everywhere. Tricky!

Superficial to deep is relative to the surface of the body. E.g. skin is superficial to muscle, bones are deep to muscle.
