In the Philippines, middle name is from your mother's maiden surname. Example. Jayson Gabawa Rodriguez. Jayson is the given name, Gabawa is the middle name (mother's maiden surname) and Rodriguez is the last name.
As an arab, I have a lot of names, my first name, father's name, grandfather's name, and family's name

If that is read as straight, i would assume that Hernandez is your Surname, and Javier Alredo is your given name (double first name), and Hernandez is your middle name. But in every country it is different so maybe both Alfredo Hernandez is your middle name or Javier Alfredo Hernandez are your first name making it a triple first name.

Basically your first name is Mohammad Irfan and you last name is Ali. You DON'T have a middle name. If you try to use "Irfan" (the name you are called by) as your middle name on the FORMS officials might omit that name OR JUST USE THE MIDDLE NAME INITIAL and use Mohammad for your first and Ali for last name. This is how they put your name on official documents most of the time in USA, CANADA and the UK.
So if you want your name to appear like IT IS then you must put Mohammad Irfan AS YOUR FIRST NAME AND ALI YOUR LAST .
