Your observations with regards to the SPLIT Trial are on point, its basically a trial showing that with healthy patients (i.e. not having deranged renal function to begin with) N/S and Balanced solutions are equivalent - a finding of very little use in the setting where I work and, I suspect, most ICU's/Emergency Depts around the world! With respect to DKA treatment in particular, I think your observations are profound. Current DKA protocols, that Im familiar with, all advocate initial high volume (often 2-3L) resus with NaCl 0.9%! Considering NaCl 0.9% has a pH of around 5.5 and, given the high rates of AKI associated with the condition, such a recommendations must surely be having a deleterious effect on patient outcomes across the world?
My mother has high ldha (around 250) Has brain inflammation hydrocephalus and neutological pain that increases when she has anger episodes... Saline solution with vitamin gives her some energy back...but her legs swelling is notuced.. Which fiuld iv solution would u recommed for viatmin c iv Hope u can help
