Vinay Prasad MD MPH is a hematologist-oncologist and Professor in the Department of Epidemiology and Biostatistics at the University of California San Francisco. He runs the VKPrasad lab at UCSF, which studies cancer drugs, health policy, clinical trials and better decision making. He is author of over 450 academic articles, and the books Ending Medical Reversal (2015), and Malignant (2020). He hosts the oncology podcast Plenary Session, the general medicine podcast the VPZD show is active on Substack and runs a RUclips Channel @VinayPrasadMDMPH. He also uns The Drug Development Letter, a must-read for industry insights. He tweets @VPrasadMDMPH.
What I found even more disappointing than the lack of debate, was the lack of research. For example: the world spent a year arguing about whether masks work, but conducted almost no experiments to try and find a more definitive answer.

Because it didn’t matter if they worked or not. Democrats “Knew” the best course of action was to write tickets to Republicans that didn’t want to wear them.
There was no research done because IF it was show masks were useless Republicans could use that argument in court.

The problem with the logic of reopening elementary schools and middle schools early is that many of the teachers at those schools were middle-aged and older, so they would have been vulnerable to the virus brought into the schools by low-risk students who weren't suffering serious symptoms but were nevertheless carrying the virus.
Baloney. The really vulnerable were people over 65 with other comorbidities.

That's a lame reason to cause entire generation of kids to forego their educations for a couple years, and very few teachers are over 65 with comorbidities..
I'm sorry but the speaker referring to "urban cities" got me wondering if he was aware of any other kind of city.
I think by "urban cities" he meant large cities (e.g., San Francisco). In the U.S.A., "city" is a kind of technical term, with many, many relatively small locations being classified as "cities" (as opposed to towns).
Small or rural cities are also types of cities so the Dr. is differentiating large ,urban areas from smaller one s.
What he really means is places ran by progressive liberals. He’s progressive, but with an honest scientific mind, so it’s hard for him to admit that his side was absolutely insane, vile, and authoritarian during Covid.
The sad thing is if another pandemic popped up tomorrow, academia, medicine, media, government, tech and elites would still go right back to the same playbook. We have learnt nothing. The same people still operate by feelings and not by facts.
This is highly likely unless we have an honest and transparent discussion around the mistakes we made. And more importantly people take responsibility for mistakes.
We only have to blame the obeying masses (or herd) of sheeps complying with the non-sense mandates and absurd rules. If the masses refuse to comply, the game and the delirium of the so called "elite" is over in minutes. This is the summary

The CDC and FDA could claim that - "reading tea leaves" and "inspecting chicken entrails" constitute - "the science" - but clearly that wouldn't make it so. The capture of "the science" by monied interests - and the political class literally "owned" by those monied interests - has destroyed anything resembling free and open scientific inquiry.
Crazy that the medical staff could go home to their families and return to work the next day. Yet the families of patients were prevented from doing the same exact thing.
I don't think enough people understand how important Dr. Prasad's talk really is in a democracy that holds freedom of speech so dear. He methodically takes us through the missteps that continue to lead us astray. Away from uncovering truth and retaining a free and informed society. I really hope this talk goes viral (internet viral). Prasad's logic and reasoning are so compelling that it boggles the mind that any educated individual would seek to destroy him just because they don't like what he says, which is basically the truth. What was rather interesting is that RUclips served me a version of this talk earlier today, which I watched. Right after I watched it it was removed from RUclips. My first thought was that RUclips was censoring the doctor or Heterodox, but I believe Heterodox deleted it and reposted it for whatever reason. It is the sign of the times that we think the worst, without evidence. Very sad.
I saw that too, and had the same initial response. I think what happened was HXA uploaded to the HXA Out Loud channel by mistake and then deleted it and re-uploaded to the main channel.
Statements weren’t checked if they were right or wrong, but if they were Right or Left.
The Lancet published an article stating that it was essential that Americans not reelect Trump because his COVID policies were so bad. This was done at a time when the UK had a COVID death rate 25% higher than the US.
Conservatives were called murderers because they wanted to go to work and earn a living.
Covid response wasn’t a “failure” of the academy, it was the academy jumping in line to do the bidding of one political party.

Totally agree with this, but it was dying relative policy that convinced me that the world has lost its collective mind. Australia did this - and you couldn't even cross a state border to visit a dying relative. And that was an explicit violation of our constitution.

They are still going after scientists and doctors today. They are trying to pass a WHO treaty that would implement all the bad policies used during covid. We are still facing the same issues.
I can cry as I hear you and breathe deeply. You speak reason that I felt and thought and believed throughout the pandemic and felt so confused by the policies and demanded behaviors to follow. I grew up in a school that taught critical thinking so I couldn't understand why we couldn't talk about the trauma that was blinding us and hold critical Debates on everything that was happening. It was a horrific example of how authoriatarianianism is a natural flaw of blind reactive human reactions.

I don’t follow prassad because I think he is always right. I follow him because 1: his logic is usually sound 2: his credentials are appropriate 3: his approach to specific policy decisions seems honest and structured 4: he is broad enough to incorporate ethics and practicality.
Vinay kept me sane during covid, while the dictator Biden and his sycophants were destroying my life!
Thanks to Dr. Prasad for speaking so eloquently on these topics. I started the pandemic with frequent doomscrolling. When I stopped believing the hysteria, anonymous internet commenters happily told me that I was listening to all the wrong people. I’m glad I stuck with people who are adherents of evidence based medicine, and I’m grateful that Dr. Prasad has persisted with his sensible messages.
There are three distinct issues - 1) freedom of speech . This is a human rights issue 2) freedom of academic debate within academic institutions. This is an issue relating to the development and sharing of knowledge. In one short talk I would have preferred to hear more about the latter given that this was the title of the talk. The stunting of research (and academic debate is a part of the research process) is a serious threat to the human species.

I have grave misgivings about the once gold-standard of Peer Review. Going against the established narrative can now lead to loss of social stature, censorship and even complete career loss.
It has always been that way from beginning of science. We want to fight against the tendency, but don't think this is something new. Nor does it mean that anything new or contraband is automatically true or accurate. Tread carefully.
Sadly, the beginning, continuation of and even very recent application and study of science has never even come close to the corruption and power of today's Government when combined with the power and corruption of social media, Big Pharma and Big Tech. The cancellation, censorship and career destruction is now quick and permanent. @@momhouser

Having said that, I agree with Dr. Prasad that there is not good evidence that masks are effective, and mask mandates are unjustified.

Academe did not host debate. Indeed. In a science writing class I attempted a debate with students on Covid policies. They were incapable of it and it resulted in intense emotion and me eventually being bullied for it. It was never modeled so my effort was too radical. This will fester

As a nurse who has personally witnessed a global propensity to neglect institutionalized elders I can't even imagine what care was like when no one was looking?
My father 64 year old was KILLED by negligence in Portugal, at the University Hospital of Porto Sao Joao, in July 2020… because of the horrific covid policies and the fact that the medical staff was completely mad.

We should have trusted in The Great Physician rather than a govt. appointed expert?????. The Great Physician had our best interest in mind rather than amounts of profits for big pharma stockholders.
Most MDs, are not great physicians. Med schools teach, diagnose and treat. They don’t have a clue, and the med schools and studies are supported by the pharmaceutical companies…..

Good talk. I think scientific questioning people got thrown in with the consume horse poop people, and the rest of us were like um, no, if there are some wackos, they are all wackos. It is such a sobering reminder (as is true in another hot, highly censored topic that starts with t and ends with r) that there is great variety of people on both "ends" of the debate.

I noticed, too, that any debate on the acceptability of using, let alone mandating, a product that was developed, tested, or manufactured using aborted fetal cell lines. I will never accept such a thing myself. Not even to save my life. The child in this case was a female aborted at six months' gestation. Not a humane event at all.

We might not have known, but the powers in charge certainly did and manipulated the situation to their benefit.
I'm not highly educated but had questions about natural immunity (and masking outdoors as well as for kids), which, for virtually every other disease, was better than vaccinated immunity. It was an innocent question, and I couldn't believe how my adult kids responded to it. Yes, we didn't know a lot in the beginning, but it seems that there were forces co-opting the whole situation for political reasons. I truly detest politicians who caved to their party lines and corporate donors regardless of the D or R after their names, not to mention the civil servants who became mouthpieces for both.
CDC study said IFR 0.4 % by mid 2020, average 2.6 co-morbidities and average age 78. The problem was that the community couldn't understand 0.4% is 4 per 1000 of people who were already in God's waiting room. Can't help stupid!!!
We knew very early thanks to Chinese research that it was a coronavirus and we knew very early that it was potentially fatal, mainly in the over 65 age group.

We have a son who have 5 kids under 8, homeschooled, mom and dad worked from home, were very protective. The whole family fot sick Covid, from 7 years, to the toddler. Don tell me younger children don’t get covid…..
I think that the point is that young children don't have high mortality and morbidity from covid.
We were extremely isolated during COVID, due to living situation, working at home, and when not working at home working jobs with minimal human interaction. We got COVID twice during that period. Our younger kids didn't have symptoms both times, but they all had mild symptoms at least once. I was knocked out for a few days, but the worst was muscle problems that came up a few days after I was over it. It took months for some of that to go away, and the second time I (thankfully) did not have that and had extremely mild symptoms.
Younger children seem to be just as likely to _get_ COVID as anyone else, but the majority have few or no symptoms, and they are typically extremely mild. That doesn't mean that younger kids _can't_ get COVID with more severe symptoms though. This is why it's so contagious among kids. Because some do get symptoms, people initially assumed that kids with COVID would _always_ have visible symptoms. It was half a year or more before people (including doctors, many of whom initially refused to accept the evidence for political reasons) started accepting that many kids are asymptomatic carriers and that at least half of the time, kids don't get symptoms when they get COVID.
It is always easy to do Monday Morning Quarterback. At the time of the panic, I don't think there was time to do a debate. How long would the debate have taken? What you expressed is the epitome of individualism, characteristic of a neoliberal capitalism. I agree the pharmaceutical companies took advantage of the panic situation of the pandemic to increase. It certainly makes no sense to vaccinate those who had been infected. That's just a way for big Pharma to make more money.

Did you listen to the talk? Even by 15 minutes in he cited multiple instances in 2020 where those with views and data, even attempts to collect data, outside the orthodoxy were silenced. “We couldn’t have known” is a bizarre cope, given how many physicians and researchers were speaking up by latter 2020.
Nonsense. I publicly argued against shutdowns and masking and forced vaccinations from day one. I knew that the pandemic would get better as the virus mutated to less virulent forms, and I knew that herd immunity would be the best plan, with elderly persons the ones who needed protection. Some of us are smart and some are not.
Learn some medical history and about the relationship between corporate media and big pharma. The lack of debate and scientific censorship and in some cases rushing through approval has caused numerous health disasters. This one just eclipses all others.
While I agree with several points made here, particularly about the suppression of debates (like lab leak), many of the decisions made at the time were based on the available evidence. Much of Prasad’s argument is based on the evidence acquired through time, retrospectively. So some of it seems unfair. In addition, variants of the virus were not all the same. The early variants at least at the time indicated higher death rates than later variants that would follow. Absent accumulated data, policy makers have to make decisions.
There was contravention evidence, quite early. And we all know it.

HXA is promoting a sloppy if sophisticated analysis here. He makes many assorted points, the vast majority of which are easily countered by anyone thinking critically. Were there many questionable aspects of decisions made under fog of a health emergency? Of course, and I want much of that to go on, just as I want decisions made with limited debate on battlefields, and by emergency rescue personnel. Its good to reflect on the decisions, but a lot of this talk seems sloppy heterodox contrarianism for its own sake🙊🙉🙈🫣

I'd say the debate shouldn't have been stifled to points of censorship or public shaming of quality inquiries, or alternatives. Some damping of media tendency to promote the most flamboyant (in times of emergency), does seem warranted. One problem though, is pulling back as emergency is less intense...

I'm not sure it was stifled as much in scientific dialogue as it was in the public dialogue. I was well aware of most all the counter and alternative views now being highlighted at the time and I'm just a curious informed outsider, that tries to get the gist of the debate. Some public figures may have been pulled too far by the media insistence on binary good/bad true/false framings... But most often even in those circumstances there was some temporal hedging in genuine authoritative public dialogue. The media and politicians are another matter.

One point that I hadn't made... Even If the damping of media amplification of more sensational and dubious is appropriate under some circumstances of time and safety..... The polarization of our society resulted in noticeable and unproductive biases in favor of some poles and opposed to others.
Ex. There was little mainstream media or health authority pushback against the far reaches of aspirational safety-ism... even after they were easily descredited.
I agree with you: if the OP thinks the overall response to c19 was acceptable in every way because of the ‘fog of the health emergency’, then he presumably is okay with us doing broadly the same next time. Well, it’s a point of view, but not one I share: I’m not sure that we can afford to make such a fk up again soon, and that particularly includes our medical professionals.
