Khadgar killed him, or at least he thought he did. In reality, Khadgar destroyed his body that was possessed by Sargeras, but Medivh's spirit escaped and with the aid of his mother's magic, his spirit managed to attain its form and later resurrected by the use of Aegwynn's last magic. However the new resurrected Medivh wasnt recognizable even by those few who actually knew him. Antonidas as the leader of the Kirin Tor, knew Medivh. He had seen him, he had spoken with him. However in the W3 missions when Medivh was seen talking to Antonidas in Dalaran, in that scene where Jaina was hiding, Antonidas didnt recognize him. So one would assume that nobody that knew Medivh before his death, could recognize him after his resurrection. Maybe Khadgar or Lothar would but they werent in Azeroth anymore.

I'm just imagining The Culling being similar to the mission in HotS where Kerrigan and Zagara race to get 100 banelings first. Garithos arrives early enough to rescue uninfected villagers. If he gets 100 of them before Mal'ganis converts 100, there's a sequence at the end where your army is backed up by 100 additional footmen to siege Mal'ganis's base. Probably too many continuity issues with that, sadly. Still looking forward to seeing how this story pans out.

Medivh will give a positive if cryptic answer and Garithos sends the Captain with a full written report about his Campaign and the Enemies Humanity faces to King Terenas. The King might threw Medivh's warning careless away, but not a warning from LORD GARITHOS with the backing of an experienced long serving Captain of his Army AND evidence about a coordinated spreadingof a Plague.
