Normally they use porcelain (China) mixed with other insulating properties to strengthen the material. If you ever were to flick or tap on these insulators, you will here a nice ring to it.

Yes, they use template for the desired shape... and it's a wet sponge used in last for smoothening the surface

A Die of that shape is attached vertically parallel to the lathe kind of machine, and die is guiding the cutter to make this shape...
