Mountains were missing from topographical chart, I think this is the one of the main reason of this crash, it made a conformation bias in captains mind that Terrain warning is false as there are no mountains near by. Indonesians called this mountain most infamous in the area for airplane crashes, still this mountain was missing from the topographical chart given to pilot, can you imagine this !
If the pilots are not taking any action within a stipulated time then the collision warning safety system should automatically bypass the manual controls and switch to Autopilot to avoid any disaster. Please redesign the Avionics safety system.
