    Back in the day....
A leaking brake booster would make the engine run rough.
Todays ECU keeps the engine running smooth.
    Good diagnosis.

Would this have been also caught with a smoke tester?

Depends where the check valve is for your vacuum source you may not be able to get smoke into the booster. It's just easier using live data and knowing what you're looking for than setting up a smoke machine.

Should the brake booster be replaced or can you live with the lean condition, is the brake pedal really spongy too?

the customer came in for just the check engine light no issues with the brake pedal. A couple of the guys have replaced a few of these on the CRV.

Is the idle smooth or fluctuate slightly?

      .

It actually idles fine bcuz the computer has already adjusted for the extra air. The idle changes instantly when pinching off the hose because it doesn't respond quick enough for the change in air/fuel.
