  • How to request credit for a part 121 proficiency check or new type rating to get a certificate showing proof of a flight review in accordance with FAR 61.56.
I would be more than glad to help my fellow brothers and sisters at the airlines, so feel free to use me as a validator and I’ll do my best to validate the credits within 24-48 hours free of charge.
Step 1: Register for an account on www.faasafety.gov
Step 2: Search for and request credit for a Part 121 proficiency check or type rating
Step 3: After credit has been validated, logon to “Pilots - My WINGS” and click on the completed phase in the right hand side. It will automatically give you a certificate showing a flight review on the 2nd page.
NOTE: Any validator you use will need evidence to support completion of the 121 proficiency check. This could be a copy of your training schedule, a picture or photocopy of your new license with the type rating (or temporary airmen certificate), or a copy of your training record. Check with your validator to see what evidence they will need.
    FOR VALIDATORS (FAASTeam Representatives):
The Activity numbers are as follows:
    121 Proficiency Check: A080118-01
    Initial Type Rating: A071102-05
    ALTERNATE METHOD:
Once you already have an account with FAASafety.gov, you can email a validator or FAASTeam Representative with a copy of your evidence and the date of completion, and they can validate the credit for you online.
