the birthday card and gift is have to be on the birthday day? Or still we can request even the birthday month?
and is there Satay for this Japanese bento box before actual meal?

It can be requested as long as your travel date is within a range of days to your actual birthday, you may drop them a request via their website to check on their latest policy but it is not a long period from memory (likely +/- 1 week).
The Satay appetizer depends on the timing of your flight, as mine was a sleeper service on a midnight flight there was none, however if it is a day flight there should be. Hope it helps!
