It’s even more sad if you know that another thing they said before the ship caught fire was “How are we going to get to the Moon if we can't talk between three buildings?” The crew knew there were multiple problems, but nobody cared. It pains me that those people’s lives weren’t focused on enough at the time.
Not only was there an oxygen rich atmosphere, the capsule was also packed with highly flammable nylon netting, foam pads, and velcro. This wasn't just corner cutting, this was completely throwing safety to the wind to the point where a deadly fire was pretty much inevitable.
Oxigen is a catalyst for combustion, but it is not flamable on its own, it allows other things to burn. The more concentrated the easier other things combust in its presence.

When something burns, a flammable substance is combined with oxygen, while releasing heat. In short, "flammability" is not applicable to oxygen, because it is a measure of how easy a substance combines with oxygen, while undergoing highly exothermic reaction. If oxygen was highly flammable, we would be in big trouble, since there is about 21% in the atmosphere.
Ego missions that succeed would make the people in charge look like innovators. It sucks that failures are the only way to expose them. We can only hope these failures aren't lethal.

I would completely agree that this is totally preventable. It might take about an hour, if not up to a few days to check on the spacecraft just to make sure everything works as it should be. It's a big mistake for NASA not prioritizing this. A one tiny mistake can ruin potentially everything, especially with expensive equipment and lives you can never revive.
