I'd put Orphan of Kos a bit lower. Don't get me wrong, he'd be scary as balls, but I feel like he'd be more on the level of a Xenomorph in terms of scary rather than Cthulhu great video none the less!
i think hes at least number 3, he looks scary af, but not that bad compared to everything else, however the moment he randomly sporadically jumps at you is the moment where you realise how fucking terrifying he is
As someone that suffers really severe trypophobia (only when it looks/is organic) seeing Ludwing and his second mouth with hundreds of eyes or the Amygdalas will definitely give me an anxiety attack so severe that I'll probably d*e (fortunately for me, it didn't trigger in game so didn't affect me as much while playing), so I 100% agree with your list.
Gehrman's doll shenanigans make him the scariest thing on this list that wouldn't have me questioning reality itself. Also, my favorite part about Lovecraftian horror is that some people are so stupid that they're somewhat immune to the scary bits, while the smarter someone is, the more likely they are to get vibe-checked by more tentacles and eyes than can be counted. It's the cleanest way I've seen to make an unbeatable source of dread.

Honestly for me there's extra tier: "existential horror". Like yeah there're some bosses that are nightmare fuel but you know what's scarier? Bosses who's mere existence proves that there's something supernatural about our world hiding on Earth. Like yeah some bosses can give nightmares by their looks but bosses that prove that there's something fundamentally wrong with this world are way scarier. They prove that there's irl eldritch shit

Cool video! I mostly agree with your list however personally I feel I’d ultimately be less scared of Ebrietas than the blood starved beast. While Ebrietas would initially be more terrifying she is also kindhearted, so after a while I’d just be excited to meet her. Meanwhile encountering the BSB would straight up traumatise me 💀
