You wouldn't really get any fallout. Nuclear fallout is caused by dirt and debris at ground zero that is heavily irradiated by the explosion and sent flying up into the atmosphere before falling back to Earth over time. Meanwhile, airbursts like this don't create much fallout as there is no dirt or debris at ground zero. However, because there are no obstacles or earth at the site of explosion, the explosion can be much more powerful and create far stronger and more devastating shockwaves, which is what causes the real damage in a nuclear explosion. If high enough, they can also cause something called EMPs, which fry all electronics that aren't under a faraday cage within...I'd say about a hundred miles. (Can't remember the exact distance off the top of my head)

Not necessarily actually, over 200 years of not being refilled most of the deuterium and tritium in the ICBM would be gone, reducing effectiveness to a fraction
IRL nuclear missiles and Fallout nuclear missiles are not the same, and not just in the bullshit video game "science" way. IRL, it was assumed from pretty early on that nukes would be used to annihilate the enemy's entire civilization. So we have ICBMs which will wipe out cities entirely. In Fallout, nukes were actually used and therefore developed, tactically. To take down troop contingents, military bases etc. So the missiles were smaller in size and blast radius, but also ofc there were a whole lot of them.

To be fair it was a big red lever, not a big red button. Nobody ever said anything about not pulling big red levers, so obviously the fault lies with the designers who did not use a common "this means bad things will happen" design for their control console. She's completely innocent of any wrongdoing.
Which is why is way more fun to watch someone who doesn't save and instead deals with the consequences. I never let myself live through bad decisions in games but when you commit to something is way more fun answering to your actions.
