Although she considers Miquella to be the strongest of all the demigods, the title of "Blade of Miquella" refers more to the fact that she has decided to fully dedicate herself to Miquella's ambitions, lending him all her power, rather than pursuing her own ambitions.
Considering Miquela's strength is in dreams and what could be done with that, i think it was said somewhere that he had the most potential to challenge the greater will itself. We don't know who either Miquella's or Malenia's shadows are, but since both are empyreans, I wouldn't be surprised if Malenia is filling the role of Miquella's shadow. She certainly qualifies.
So Max0r didn’t cover it but Malenia phase 2 comes packing with Scarlet Rot which made Caelid so infamous a level. This translates to her phase 2 being the same as phase 1 with flight and the ability to throw out nuclear blasts complete with radiation poisoning
Remember how Starscourge Radahn's mind had been eaten away by the Scarlet Rot that also turned Caelid into the land of German bedtime stories... that was because Malenia entered her second phase while fighting him. True horror is a fucking understatement.

I have died to Malenia 149 times before I finally beat her on the 150th fight. Yes I kept track. (to be fair I relied on summons my entire playthrough and when I got to her I wanted to beat her one-on-one, so I kinda deserved that) I have heard "I am Malenia, Blade of Miquella" so many times I opted to mute all voice audio before it drove me insane.
It even worst for people like me who got the game near launch. Given even with summon she wreak shit out you. I died her like 250+ times and I’m not only one during that time. She definitely gotten the title of hardest and max0r isn’t kidding of the smallest mistake she punish you hard.

I only beat Malenia because of a glitch that left her on one health at the start of phase 2... I didn't know that glitch existed and it was patched out only days after I beat her. I beat the second hardest boss I had ever fought in my life (only second hardest because Kingdom Hearts 3's DLC was insane) with a glitch I didn't know existed so I was so confused to the point where I just stared at the screen for 5 minutes, unable to comprehend what had just happened. This already makes me so excited and scared for the Shadow of the Erdtree DLC, Fromsoft always finds an excuse to stuff the hardest thing they've ever created into their newest DLCs. And no doubt, there's going to be just as many people complaining about the hardest boss in the DLC as there were complaining about Malenia (even though her difficulty completely depends on how quickly you can learn her patterns, just like every Fromsoft boss).

That being said, Max0r decided to mod the fight to become even harder and used some footage from that fight in this video. Then he spent an entire stream trying to beat the modded version. He did not succeed. As for her brother, he is actually going to be focused on in the DLC when it drops this month. As of right now he's trapped in a cocoon by his half brother Mohg so he can do incestuous things to him, hence the Lord of Incest. The game's backstory is written by George R R Martin, of course there's gonna be incest. Makes it easy to hate the bad guys more. Thankfully, we killed Mohg in the last part. We have to wait for Miquella to come out later.
Miquella and Malenia are children of Marika, making them both Demigods. However they also have curses. Malenia's is being a vessel of the Scarlet Rot, an outer god. She's the reason Caelid is such a hellscape. Miquella is her brother, and his curse is eternal youth, IE his body is that of a child, permanently. His primary goal is to cure his sister's Rot. Miquella is the hand in that cocoon you see at the end. He was ripped from the Haligtree by Mogh who is his half brother, and after we defeat Mogh he seems to be dead. Miquella is going to be a vital character in Shadow of the Erdtree, the first DLC for Elden Ring which is coming out soonish. So as a result of that we naturally don't see much of him here. We do know he's not really a fighter though, in fact he's primarily characterized as being incredibly nice and empathetic. He pretty much just wants to make the world better, which uh.... Yeah this is a From Soft game, easy that is NOT.

In defense of people that say Malenia is the hardest, she is probably the least fair fight in the series. Max0r wasn't kidding when he said that you just aren't getting away from Water Fowl Dance (the sword attack in her first form where she slashes everywhere). The attack goes on forever and it will track you down to the end of the earth. No amount of dodging is going to save you unless you are able to hit a perfect 3 frame window multiple times in a row, and running away from her is basically just a coin flip on whether you take a little damage or flat out die. On top of that her health steal every time she hits you, including when you block, invalidates a lot of builds with the intent of tanking or brawling with her. Her second form is even worse and he kind of glosses over it in the video since most people watching are familiar with her bullshit. She starts the fight diving right at you before the cutscene is fully finished transitioning back to gameplay so you are 99% guaranteed to die the first time you get to that form for starters. On top of that she's far more mobile and has massive AOE explosive attacks, as well as the ability to summon clones of herself that will hunt you down with their own BS nearly unavoidable sword attacks. ALL OF THAT WITHOUT EVEN THE MOST AWFUL PART OF THE FIGHT. That being that everything she does inflicts and creates Scarlet Rot, which drains your health massively over time the longer you're in contact with it. It honestly feels like Malenia is a boss that would fit in better if she was put in Sekiro where faster gameplay and a ton of parrying attacks and juggling annoying debuffs is more of the norm. Which is funny seeing as the Demon of Hatred in Sekiro much like Malenia feels like a boss that was designed for a game other than the one they were put in (in DoH's case Bloodborne is where is he'd fit much better with the play style of the game).
Yeah... Malenia is... pain... much... much pain... I have seen entire 4 hour streams go just trying to kill her, and only getting to Phase 2 which is much... MUCH worse since she opens it up by blooming a giant flower that deals damage AND procs Scarlet Rot which is a combo that SUCKS... and then there are the hallucinations she summons for one of her moves... yeah no, she is BEAST.
Malenia is a very hard boss, however she can be cheese by bleed build weapon art spamming. So the “no she isn’t” joke is about her being 50x harder because he’s not doing a bleed build.

Malenia is completely blind which is why she waits for you to move so she can hear you and know where you are
