Hello Phyllis. The medical diagnosis doesn't matter. What are the patient problems? That's what you should focus. Most patient in the ward will have sleepless night, inadequate knowledge about condition, inability to do a simple task. These are the important things you need when writing a careplan.

No please... "Patient can complain of being unable to eat." The reason is that, the patient is not a health worker and it is assumed the patient doesn't know the medical terms. So patient complains is written in "plain" terms. However you can write as part of your objective assessment that, "Patient is anorexic" I hope that answers your question please
