Is there a racial bias for foreigners in Tokyo nightclubs? I only ask because i've heard stories of Indian , Turkish, etc tourists being denied entrance into some nightclubs in Tokyo, an issue that wasn't enforced upon caucasian tourists.
Yes. Although most places in general are anti foreigner irregardless of race some are discriminatory towards darker skin people. The irony with Asia is that we do not discriminate against them in the west but they do it to us in the east. A problem that needs to be addressed.
If there are nightclubs that deny foreigners, the reason is probably no staffers speak English or any foreign language, thus they can't communicate with foreigners(everything on the menu may be written in Japanese). Or they may have had bad experiences with foreigners with bad manners (especially towards female customers) before and never want to let foreigners in again. But if you are able to communicate in Japanese and are well-mannered, you'll probably be welcome.
I had found Club Camelot, Warp and Atom were my favourite clubs and foreign friendly whilst I was visiting Tokyo, great vibe and great music. I have heard from friends of mine that ageHa was the best for them, unfortunately, it permanently closed before I visited Tokyo
Great list, man. I've spent too much here in the last 3-weeks. There are a few smaller underground/secret entrance rave clubs, too, which are a fantastic time :)

It depends on the foreigner. Not all foreigners are created equally despite the money they may posess

Most of Tokyos nightclubs have a ratio of 60% men and 40% women
But I know that raise Tokyo is a favorite for Tokyo ladies who wanna dress up. The VIP is also very nice but check the prices
Is it right, that in WARP you are not allowed to show your tattoos?
You can actually go into WARP with tattoos. They are much more flexible, but always a safe bet to cover some of it up if possible.
