I love this idea, I want to make this to cover on the top of a blouse that fits me too low. ¿How can I saw it to the blouse if the top is above the blouse line? Thanks

I have mine positioned right along the neckline. However you can lift the whole crochet collar up to about half way. You can sew the top of the shirt's neckline to the middle of the collar, which should give you half of the lace collar on your skin.

Try sewing row 2 to the edge of the blouse so one row extends above the edge of blouse. You could also make the initial granny one row bigger so the piece would be one row wider.

Absolutely! You can sew it however you wish. I have a tendency to sew the least amount that I have to to get something to work, but feel free to do more than I did in the video.
