Been to the US many times on a B1/2 visa but never applied from Pakistan before. You would have to apply for a new visa with all new documents. Also keep that in mind US will check your social media and RUclips account and as you have been saying that you're planning to move overseas with family this point will go against you. The US will only issue you a visa if they know you will forsure come back and not stay there. You really have to plan well. Also Indonesia will not issue you a visa on arrival if you have a US visa, mostly South American and Eastern European countries will accommodate you and Turkey. But not Japan, Korea and Indonesia. Serbia is not very good country for South Asian so I would stay not bother, Malaysia is good if you got a good job or start a business but you will not get a passport there very soon, Bali is same as Malaysia but you can get Indonesian passport after 5 years, Singapore you can start business with 50,000$ and you can get PR and passport in 3 years. Good luck.
You have to apply for an altogether new visa and have some good reason as to why you never visited. You need to visit at least once to renew your visa. You can always apply for investor visa for USA which does not lead to residency and is there for 10 years. Visa B1/B2 is always for 5 years
It is called E2 visa - you can get in touch with any Pakistani immigration lawyer in the US for more help. Basically, you need to invest in an existing business or buy a franchise (80,000-100,000 investment). I am not an expert on it but I am sure any immigration lawyer would be happy to help. Search for a lawyer who talks for free on the first session. Once you have the information, then you need to connect with a franchise specialist who connects you with many franchisees - find a suitable business for you like painting, cleaning, or any small business that you can invest in. That is a good clean way to get the right visa for the entire family

For renew B1B2 Visa only fill the D160 form, Attach the photograph of white back ground and paid the fee. I think now fee is 160 USD.
