So instead of problems being addressed and fixed people instead play the blame game and focus on coverage meeting issues with the organization and procedure don’t get remedied, so problems go on solved. A good example of this in the modern day is actually the military, which takes a very punitive attitude towards any and all incidents or reported issues, regardless of whether or not, they were one’s fault. This is especially true for higher officer, promotions, where there are only limited slots and limited windows, in which an officer is eligible for a promotion, instead of having to retire under the up or out system. So the officers who actually get ahead and make it to positions like Colonel and General aren’t necessarily the best officers at their jobs, but rather are the best officers at limiting liability and incidents. They know to avoid any bad press, and know how to make themselves look good, regardless of whether or not, they actually produce performance and results that they are supposed to.

After a few weeks he actually asked Palpatine that question. The Emperor's reply was " that it was what the smelter was using that day.
I dunno ... It starts to feel old that people are still calling exhaustion port, that works exactly as planned to be "construction flaw" ... Maybe designers simply didnt count with space wizards capable of changing trajectory of torpedoes with their Force shenanigans in a way that defy physical laws ... one could even argue that it wasnt exactly smartest move to erase any evidence of their existence. :-/
Exactlt this. I like it so much more as this being interpreted as Palpatines self-destructive nature which eventually lead to his own downfall. Underestimating Darth Vader, erasing evidence of the Jedi and thus giving them the chance to rebuild their power, cruelly punishing the Death Star engineers over what is just a freak accident. His sadism has always been his undoing.

Yeah. The idea that the entirety of the Death Stars waste exaust was funneled through one 2 meter wide hole is actually an engineering feat. Like you pointed out, people forget Luke literally used the force to curve the Torpedos into the exhaust port. NO ONE could have predicted that. This is just Palpatine being needlessly cruel over a freak accident, which unironically demonstrates the Empires failure.

Hey Stupendous Wave, while this is bad, it's just torture. I've always found that what is worse than doing evil is to force others into evil. I don't just mean corruption like with Vader. I mean making a pawn of someone who doesn't realize they are a pawn, setting them up to do things they find atrocious but somehow can justify it to themselves, only to find out it was all for nothing BS that you tricked them with and got them to do terrible things. That's worse and I could see palpatine doing this. On an aside, did you know Darkseid was the one who corrupted Dasaad into the torture loving sadist he is today and Dasaad was once a kind lovely child from New Genesis not Apokalips? Hey Stupendous Wave, while this is bad, it's just torture. I've always found that what is worse than doing evil is to force others into evil. I don't just mean corruption like with Vader. I mean making a pawn of someone who doesn't realize they are a pawn, setting them up to do things they find atrocious but somehow can justify it to themselves, only to find out it was all for nothing BS that you tricked them with and got them to do terrible things. That's worse and I could see palpatine doing this. On an aside, did you know Darkseid was the one who corrupted Dasaad into the torture loving sadist he is today and Dasaad was once a kind lovely child from New Genesis not Apokalips? At least Vader takes no pleasure in his evil but Dasaad is in some ways even more depraved and vile than Darkseid.
