The largest scale effect was the smoke inside a building. How about larger than that? How about a foggy forest or a sandstorm?
You might be able to simulate large areas of smoke if you will have large voxels. But for fire you usually need bigger detalization and as a result smaller voxels. Also, you can have small simulation area and just move simulation volume with your character so you won't waste resources for computing smoke far from the camera.
