How to Use a Custom Functional Capacity Evaluation Form to Win Your Disability Case

- Since Social Security defines “disability” in terms of how your medical problems and associated treatment (like medication side effects) prevent you from performing the duties of a simple, entry-level job. A detailed functional capacity form completed by your doctor can be an indispensable tool to present to the Social Security judge.
In this video, I show you the official #SSA functional capacity form and I explain why this form is flawed and won’t help you much. I also show you an example of a custom form that addresses the specific work activity issues relevant to a Social Security judge.
In my practice I would customize this “custom” form even more to make it specific to your medical records and you exact situation but even in its unmodified state the custom form I found online is far superior to SSA’s official form.

I received a funtion report questionnaire. Just reading the first few questions gave me a migraine. I was going to withdraw my claim before I watchied this video. I got it out the trash and will follow your valuable guidance to fill out & send back. Thanks for all you do.
It is so hard to get doctors to fill out these forms. Tbh I think a lot of them don't know the answers to all these detailed questions which makes it harder. Plus they don't want you disabled they are trying to get you better, so a lot of times they'll get weird when you ask.
I handed this form to my doctor and she looked at me like I lost my mind. I know my diagnosis and symptoms, but even I became overwhelmed when reviewing the questions. Basically, its asking the doctor to do a college exam, they've got to dig up the medical evidence, diagnosis and physical findings.🤔
You must have your doctors fill these out. It is imperative to have these RFC forms for your law judge hearing.
I printed one off a an online attorney's website, he had free Eval Forms for various illnesses, mine was specific for Parkinson's, I handed it to my Neurologist, she filled it out right there. I know this is the biggest key, I'm hoping to apply and get approved off the bat, but I'm being realistic. Soon as today's dictation comes back I'm ordering med records and applying in person at local office. I got all records from my treating physicians my self. Hoping for the best.
where can i find that custom form you were showing. that practically perfect. I thought about crafting a form with my lawyer to bring to my hearing.
I have a question about RFC forms. Since the 2017 ruling on medical evidence weighting can a judge really just disregard these forms altogether if they so choose or would that be a reversible error? I have multiple forms of this kind, plus a functional capacity evaluation saying full-time work is beyond my capacity but my judge approves almost no one (14% approval). Just wanting to know if I should expect to have to appeal.
You can certainly make an argument that the judge did not act reasonably in giving no weight to multiple FCE forms. are you saying that SSA will accept a "custom" form in place of form SSA-4734? If I find a form online that fits my limitations, I can submit that form instead of the standard form, correct?

Based on the level of impairment you describe I would think you have a very good chance at winning. I certainly hope you do.

Hi I have a question I called social security 800 number and the lady told me they have until April 6 to give me a response on weather or not I'm approved or denied then she said if you don't get a check or letter by then call us back is that routine talk or does it sound like I won I'm only at the initial stage but my doctors wrote letters saying I meet listing 4.05 recurrent auhrithmias plus other heart condition and health issues they filled out these forms all in my favor does it sound like a winning case
I'm at the hearing stage my Cardiologist my treating doctor recently filled out rfc paperwork from my lawyer and even showed thru my records he gave notes details far as tests and things done medically to back up his opinion I was denied at the beginning and appeal stage is It to late to present this at the hearing stage or is it okay?

You can give it to your doctor and have him either return to you for filing or the doctor can send in directly to SSA - make sure to give the doc the correct address.

will these RFC forms either "mental or physical" have more weight if the applicant's spouse, treating doctor, therapist, caretakers or any supportive staff co sign these forms?
The RFC forms need to be signed by a treating doctor, ideally a specialist. SSA will assign no value to an RFC filled out by a spouse or caretaker.
