  • Demonstration of iontophoresis with an external phoresor.
    BEFORE USING IONTOPHORESIS, be sure to have an order for the medication and check for medication interactions and contraindications. Also check for all iontophoresis precautions and contraindications, as listed in therapy textbooks such as:
    Bellew, J. W., Michlovitz, S. L., & Nolan, T. P. (Eds.). (2016). Modalities for therapeutic intervention (6th ed.). F.A. Davis.

    Thank you for the video very informative. I use iontophoresis treatment also with the Dermadry Device, It works for me.

    How I decided where I have put the negative and positive electrodes.. which one on the active and which one on the passive(inactive)

      It depends on the polarity of the medication ions. You put the medication electrode on the lead wire that has the same charge as the ion.

    How many sessions per site ?
    I meant for example, for Carpal tunnel syndrome, how many sessions per week and what's the total sessions number that beyond it the treatment regarded as Failure ?

      That is a question that is answerable by the evidence - I'd suggest you start by searching pubmed.ncbi.nlm.nih.gov/clinical/ . If you are using dexamethasone, the biologic half-life is about 48 hours, so 3x/week would seem appropriate--any less than that and it's like taking the occasional ibuprofen rather than taking a course of scheduled anti-inflammatories. With any intervention, if you have not seen improvement after 2-3 sessions, it's time to move on to something else. 8-12 treatments is generally considered a full course, but again, that is a question for the evidence.

      Iontophoresis can "push" a medication just subcutaneously. Any deeper penetration would be by osmosis and local circulation. I would be doubtful of its role in myositis ossificans.

    Hi, Is the iontophoresis method same effective and deep as intraarticular injections and phonophoresis?

      It is not nearly as deep as injection can be, not does it get nearly as much medication to the target as injection will. It seems to make it through the dermis to the first subcutaneous structure, and no further. It would not travel intraarticular.
      This article hints at possible better results with phonophoresis than iontophoresis with carpal tunnel syndrome, but is inconclusive: Assessment of phonophoresis and iontophoresis in the treatment

    What's the difference between using this and building an iontophoresis machine by connecting a few batteries to water for a few dollars?

      Accuracy, which is pretty important if you are delivering a prescription medication.

      I have heard of saline sometimes being used...on the cathode (black lead wire) to soften scar. I'd suggest you check out the evidence on that before doing it. a quick pub med search should pull up something.

    Any specific steroids used in iontophoresis ? Or we can use ordinary steroid ampoules ?

      This is an area of emerging research. I would refer you to Bellew, J. W. (2016). Clinical electrical stimulation: Application and techniques. In J. W. Bellew, S. L. Michlovitz, & T. P. Nolan (Eds.), Modalities for Therapeutic Intervention (6th ed., pp. 287-327). Philadelphia: F.A. Davis, or a similar source. Also, any medications would need to be ordered by a medical practitioner with the appropriate license.

      Phoresis units are sold by medical equipment companies to licensed personnel. Try one of your favorite medical supply companies.

    How is this different to a TENS machine?

      Iontophoresis uses direct current. A TENS unit typically uses pulsed biphasic current.

      @@Dr.B_OT Is the current type essential to treatment? Or could similar results be achieved with a TENS machine? Trying to do some research before justifying spending hundreds of dollars on a machine marketed for iontophoresis vs a much cheaper TENS machine.

      @@SpaghettimanJohnsonJr. Iontophoresis and TENS are used for two different purposes. Iontophoresis is used for transdermal drug delivery, while TENS is used for pain control without medication (through a variety of mechanisms, depending on the settings you select).

      Ahh - that is a really good question. It seems to make it through the dermis to the first subcutaneous structure, and no further. Given that there are capillaries in the dermis, yes, it does reach the bloodstream. How much and how quickly it reaches the bloodstream is really an unknown.

      Studies have shown that tap water Iontophoresis can be used for treating palmar-plantar hyperhidrosis in patients with a pretty high success rate. Might be something to look into !

      Yes it does for hands and feet excessive sweating. You can check with a physiotherapists for treatment.

      Only if BOTOX has an electrical charge (and I don't think it does). In any case, you would not want to. BOTOX requires much more precise injection technique than ionto would provide.

      No, it's pain free, sterile and non invasive treatment
      But there's risk of chemical burn Beacuse of direct current not Beacuse of ions

      I'm currently under treatment now.
      It may feel like little pins and needles kind of feeling, but it does not hurt.

      That's an interesting question! This was filmed before CoVid19, so no, I did not wear PPE for that purpose.
      The other question is whether someone should wear gloves to apply a medication to the electrode. In the video, I was just using saline, so there was no need. But even in the clinic when we used dexamethasone, we did not wear gloves. Do you think one should?

      @@Dr.B_OT Yes because there is a lifetime maximum exposure level with Dexamethasone. No sense inviting problems.
