Jai Hind Sir, I fell down on 02 Aug 24 and my MRI report states :- 1. Complete tear of supraspinatus tendon. 2. Complete tear of subscapularis tendon. 3. Medial subluxation of long head of biceps with edema & mild fluid around the tendon. 4. Mild gleno- humeral joint effusion extending to biccipital groove. 5. Tiny degenerative subchondral cyst at the posterior part of humerus head. 6. Minimal fluid in subacromial subdeltoid bursa. Since then I am advised for physiotherapy ( SWD, IFT & TENS) for 3 + 4 weeks then for Arthroscopic surgery. Sir, please guide me whether in this case physiotherapy is required due swelling inside shoulder. How many weeks I should wait for a successful arthroscopic surgery.

My MRI shows complete supraspinatus tendon tear. On the other hand I can do from raise and lateral raise (abduction) without any pain. I had shown my MRI to various orthopaedic surgeon and they recommended surgery. Whereas HOD and professor of orthopaedic at Dr. RML hospital who asked me to do certain movements and advised for physiotherapy and exercises. I regularly do IR, ER, Abduction and other shoulder exercises including dead hanging for 2/3 minutes regularly and there is lot of improvement. Please advise

Mre MRI report MRI SCAN OF LEFT SHOULDER MRI was performed on a 1.5 TESLA whole body MRI Scanner. Oblique coronal T1, FS PD & TSE T2 weighted scans of the left shoulder were obtained. These were correlated with sagittal T1 & FS PD weighted scans. Additional axial FS PD / T2 weighted images were also obtained. There is subtle depression with marrow edema noted in the FINDINGS: posterosuperior aspect of humeral head consistent with the Hill-Sachs equivalent lesion. There is tear of anteroinferior labrum at 4 to 6'O clock position. Mild glenohumeral joint effusion is noted. Mild to moderate subacromial-subdeltoid bursitis is seen. There is a type-l acromion, with a mild lateral downslope and the acromiohumeral distance measures 8mm. The coraco acromial ligament is normal and there is no evidence of any acromial spur. The acromio clavicular joint appears normal and there is no evidence of subchondral erosions. The supraspinatus, infraspinatus and teres minor tendons appear normal. The coraco humeral distance measures 9mm and the subscapularis tendon shows normal expected signal intensity and morphology. The superior glenohumeral and coraco humeral ligament appear normal and there is no effacement of the fat in the subcoracoid fat triangle. The capsule in the axillary recess also appears normal. Mild fatty infiltration of the supraspinatus, infraspinatus and subscapularis muscle bellies is seen (Goutallier Grade-I) without loss of bulk. There is no glenohumeral joint effusion. The head of humerus shows normal congruence in relation to the glenoid fossa. The articular cartilage lining the contiguous surfaces of the glenohumeral joint reveal normal thickness and signal intensity. The biceps labral complex shows a type I morphology. The glenoid labrum is intact. The long head of the biceps tendon shows normal signal intensity and morphology. The middle glenohumeral ligament appears normal. The anterior and posterior bands of the inferior glenohumeral ligament appear normal. Please help me doctor mujhe surgery nahi karana

synovium, subacromium bursa , acromio-clavicular joint region, no free fuid in gleno-humeral joint,
labrum region,
right inferior glen0-humeral ligament, labrum, supraspinatous tendon with synovitis,
bursitis, oedematous supraspinatous tendon acromio-clavicular joint secondary to post traumatic injury
may cause future recurrent subluxation / dislocation of gleno-humeral joint suggested follow up...

MRI SCAN OF LEFT SHOULDER
MRI was performed on a 1.5 TESLA whole body MRI Scanner. Oblique coronal T1, FS PD & TSE T2 weighted scans of the left shoulder were
obtained. These were correlated with sagittal T1 & FS PD weighted scans.
Additional axial FS PD / T2 weighted images were also obtained. There is subtle depression with marrow edema noted in the
FINDINGS:
posterosuperior aspect of humeral head consistent with the Hill-Sachs
equivalent lesion.
There is tear of anteroinferior labrum at 4 to 6'O clock position. Mild glenohumeral joint effusion is noted.
Mild to moderate subacromial-subdeltoid bursitis is seen. There is a type-l acromion, with a mild lateral downslope and the acromiohumeral distance measures 8mm. The coraco acromial ligament is
normal and there is no evidence of any acromial spur. The acromio clavicular
joint appears normal and there is no evidence of subchondral erosions. The supraspinatus, infraspinatus and teres minor tendons appear normal. The
coraco humeral distance measures 9mm and the subscapularis tendon shows normal expected signal intensity and morphology. The superior glenohumeral and coraco humeral ligament appear normal and there is no effacement of the fat in the subcoracoid fat triangle. The capsule in the axillary recess also appears normal.
Mild fatty infiltration of the supraspinatus, infraspinatus and subscapularis muscle bellies is seen (Goutallier Grade-I) without loss of bulk.
There is no glenohumeral joint effusion. The head of humerus shows normal congruence in relation to the glenoid fossa. The articular cartilage lining the contiguous surfaces of the glenohumeral joint reveal normal thickness and signal intensity. The biceps labral complex shows a type I morphology. The glenoid labrum is intact. The long head of the biceps tendon shows normal signal intensity and morphology. The middle glenohumeral ligament appears normal. The anterior and posterior bands of the inferior glenohumeral ligament appear normal.
