Logically, in theory the larger end should be on the side with the water flowing out and the smaller end to the other side. This in turn has no flow disruption, no direct leaks, and for future longevity if water had debri or small twigs and leaves it wouldn't get caught or snagged on inner lip of repaired pipe causing piping to block which in some cases can happen! Good simple repair video though

It started out good, but it was all for nothing ,when you put it back together the wrong way ,if you're going to show how to do it, then do it right !
Sometimes, u can not simply turn off the water. Just imagine if the source is from the bottom of a big tank full of water, whereby there is no isolation point. U need to drain all the water in the tank to fix it. This guy gave us an alternative to fix it without draining out the big tank.

Turning off the water supply while repairing is an easy job! Why don't turn it off while repairing the leak to avoid water wastage. He can still demonstrate or show as the technique by closing the main valve to avoid the water to spill on the ground. "Water Conservation!"

It seems like it would be easier and faster just to find the shut-off valve and shut off the water, cut a small section out of the pipe where the hole is, then use a coupler and some pipe glue to patch it. This seems like the hardest way to do it.
I think the second alternative is probably the better in most situations. A decent temporary fix, to allow time for a planned shut off. The first altert is more of a permanent fix, but while the pipe is cut, there's a lot of water flowing, I think the patch up is good for a quick fix. Then you can prepare the replacements parts for repair. If possible then do a repair with water shut off. If you can't shut off water, then the first alternate is a decent method to replace the patch, but first you need to prepare a drain for the water.
Yep, im gonna do all that with the water running, wife screaming, pets tracking and kids playing. While thinking about replacing everything the wayer ruined.

It's not impossible to repair pressurized water lines but it certainly isn't a pleasant experience.
