Claim creation and submission to insurance

In this video I briefed how we can create a claim. While we are creating a claim we need the information of the patient whom claim is being created and provider who is rendering the services and billing provider as well. Verily, the claim is created on HCFA form. which is named as CMS 1500 also at the backend.
I have discussed how we use coding on HCFA form. CMS 1500 is also shown in the video.
Further, I elaborated the importance of Cleaninghouse in submission of the claim. First, claim goes to clearinghouse, and after getting passed by that, goes to insurance for processing.
There are two ways to submit the claims. The first one and convenient way is electronic claim, and the other one is paper claim that can be sent on mailing address or via Fax.
