truncal vagotomy

Complete transection of the anterior and posterior vagus nerve trunk at the gastroesophageal junction, from which the vagus nerve travels through the stomach, liver, gallbladder, pancreas, small intestines, and proximal large intestine, resulting in denervation of the them.
Vagotomy usually cuts one of the branches of the vagus nerve that communicates with the stomach.
It is done for patients with a severe peptic ulcer disease to reduce stomach acid secretion.
(Effects of vagotomy)
• gastric stasis
• reduction of parietal cell response to gastrin and other stimulants and total acid secretion
• reduction of peristalsis
(Additional procedures)
To improve gastric drainage:
• pyloroplasty: A surgical procedure to widen the pylorus to facilitate the passage of stomach contents into the duodenum more efficiently.
• gastroduodenostomy: A surgical procedure to create a connection between the stomach and duodenum, used for stomach cancer or a malfunctioning pyloric valve.
• gastrojejunostomy: A surgical procedure to create an anastomosis between the stomach and proximal loop of the jejunum. Preferred for patients with gastric outlet obstruction or a severely diseased duodenum.
(Side effects)
• dumping syndrome: A group of symptoms, such as diarrhea, nausea, tachycardia, and feeling light-headed, or tired after a meal, that are caused by rapid gastric emptying, where food moves too quickly from the stomach to the duodenum.
• delayed gastric emptying (gastroparesis)
• diarrhea
• cholestasis
• gallstones
• weight loss
• bleeding
• injury to the stomach, esophagus, and/or major blood vessels during surgery
• hernia
