Do we have Root-Knot Nematodes? What killed the plants?

Root- not todes are terrible. But we had a soil test. So that was not the problem. Root rot can be a devastating for lavender too. And we have had weeks and weeks of rain. Or was it the supposedly biodegradable plug casings?
