I think that's actually because of how No Mercy was very rushed to completion leading to several weird things, most famous of all being the infamous bug that would randomly erase your data. It's likely that character slots were changed around late in development during which the story mode was already programmed, which leads to Steven Richards being a main eventer because why not. While it's technically an oversight, it's so entertaining that it ends up being more fun than annoying.

Tiny nitpick, but you failed to mention that in SVR2006, if you chose to manage Shelton, then at the next PPV Unforgiven, you will face HHH for his World Heavyweight Title. However, if you chose to stay backstage and turn heel, then you will face Shelton for his Intercontinental Title at Unforgiven.
You also have the option to turn heel in the Smackdown story mode as well. Actually there are two ways. One is after winning the Royal Rumble match and Teddy Long returns with John Cena as WWE Champion. That's after choosing to team with John Cena during the rising point of the "Who Run Down Teddy Long?" storyline. The other is after winning the Royal Rumble with RVD as WWE Champion but that option was given to you in the middle of the game's ECW Invasion angle.
I actually turned heel in 2K16 with The Authority, but i legit can't remember anything major happening, aside from being able to get the remaining managers. Anybody else?

Hate to be "that guy" but 2k16 isn't the game with the most heel turns. Every month in the original SmackDown Vs Raw gave you the chance to turn heel, usually by being an asshole. You could walk into the women's locker room and refuse to apologize, attack your upcoming opponent backstage, talk trash to Rey Mysterio after he shows you respect, try and force Stacy Kiebler to be your manager, and so many more. It even straight up tells you that you're a heel since you'll gain momentum in matches by doing underhanded tactics, like refusing to let go of a hold during a rope break, raking your opponent's back, etc.
These old wrestling games look like they have a lot more choices, customization and more fun than the new 2k wrestling games. I just watched wrestling vids because I'm planning on buying a wrestling game on the ps4 but I can't decide on what to get haha.
