The positive crankcase ventilation (PCV) valve is responsible for managing the pressure within the crankcase of the engine. Excessive pressure can interfere with efficient operation. If the valve is clogged or otherwise damaged, you may experience poor running conditions in your vehicle. This video shows you how to install a new PCV valve in your 2010-2015 Toyota Prius.

So you CAN replace PCV valve from top. All you have to do is remove five 10 mm airbox nuts, remove airbox. PCV valve is right under 2nd from right intake manifold plenum chamber. You need 22 mm or 7/8 inches deep socket 3/4 inch drive. It helps to disconnect throdle body wire connection and hose and bracket of the sensor at 2 o'clock position of the throdle body. The hardest part is to keep the 22 mm socket on the pcv valve since the pcv valve has a thin lips for the socket and a long neck.
One thing I don't get is why not access the valve from the top??? It seems much less work to take off the air intake box which takes no more than 5 min, and you don't have to jack up the car. I have not tried it from the top, but don't see a reason it wouldn't work.

At about 120,000 miles it might be a good idea to replace the PCV and clean the EGR and also install a catch can to the PCV- Intake circuit. Nuts about Bolts has a great video on Catch Can for Gen3 Prius. 2010-2015.
I changed mines. How can I tell it is working properly?

You can replace it from top too. All you have to do is to remove five bolts holding the airbox. Remove airbox. PCV valve is right under 2nd from right intake manifold plenum chamber. It helps to disconnect throdle body wire connection with a needle pick. Disconnect the hose and bracket of the sensor unit at 2 o'clock position from the throdle body.
