    The Health Center Program To become a Federally Qualified Health Center, you must apply for a New Access Point or Service Area Competition grant. SAC grants are year-round, but NAP grants are less frequent. NAP grants for Fiscal Year 2023 are expected to release in Q4 2022, but HRSA has kept the release very well hidden. NAP and SAC are extremely competitive, as roughly only 10-15% are funded. However, becoming a Look-Alike is not competitive. Look-Alikes are evaluated based on their own standards through Quality Assurance/Quality Improvement (QI/QA) plans. Although Look-Alikes do not receive the annual $650,000 award amount, they still carry the same administrative burden as FQHC NAP/SAC grantees. Since HRSA does not have to pay the award to Look-Alikes, they have set no official limit on the amount that can be approved.
    You do not have to offer all required services at your facility, and you may utilize your current referral partners. However, the Health Resources and Services Administration requires formal documentation of the referral relationship. This is typically executed through a Memorandum of Agreement or Memorandum of Understanding that describes services rendered and binds the referral partner to the Health Center’s Sliding Fee Discount Scale when seeing Health Center patients. The good news- most hospitals have charity care policies in place and follow similar guidelines to FQHC/FQHC-LAL.
