Once the agent is deployed on the host machine, how is it kept up to date? For e.g. if the agent is deployed on a Windows Server, how does it get patched if there is an update to the ACC-V plugin? Is this something that is done automatically through the instance (via the MID server), OR, would you need to rely on the server team to update/patch the agent?

Yes it can. You can setup a mid server with a public IP Adress and point this IP to be used by your agents.
