Regarding Joe's movements on stage, I found this on a website:
"Flailing his arms about, and playing air guitar, became a standard trademark in early Cocker performances. Many assumed he must have been either stoned, or crazy. Actually, it was just his way of feeling the music. A singer, he didn’t have an instrument to play while on stage. In an interview with The Guardian, he explained, “I guess that came with my frustration at never having played piano or guitar. If you see me nowadays I’m not quite so animated, but it’s just a way of trying to get feeling out - I get excited and it all comes through my body.”For the whole article do a search for "10 Things You Didn't Know About Joe Cocker."

I suspect that he had scoliosis, maybe undiagnosed. When you see him from the side, the curvature of his spine is quite pronounced, it may be the reason why his ribcage is prominent and his movements so stiff. I'm not a doctor, this is just my guess. And I seem to piss people off when I mention it.

Joe Cocker was brilliant at letting himself be moved by the music. His body, his hair, his voice - all of it was organic and sincere. If your aren't seeing a performer immerse themselves like this, that's how you know the song is shit. Joe was covering a Beatles song and doing it with more passion than any other Englishman did to that point. Joe embodied soul and oozed utter joy. Not many have the "stereo voice" which is why those who do have it are absolutely amazing and memorable. Other stereo voices are Roy Orbison, the Righteous Brothers and Brenda Lee among others.
