Sam in cage with dummy: depressed, trapped in a situation he wants to escape, all of his actions under surveillance, living in an environment where you're consistently walking on egg shells, lays in bed awake thinking about the toxic relationship, while his partner is a dummy literally and metaphorically.
Sam posing for plague doctor: plague doctor mask represents the plague, disease, death, toxicity which might allude to a toxic relationship, but he's all dolled up in his heels, coat, sunglasses. Sam realizes his self worth and self love and he vogues the house down with his dancers with the plague doctor gone.
Sam out of the cage: freedom to dance and move, dances above the cage knowing he's in a better position, happy and smiling and living his best life, while partner is still trapped in the cage.

Listened to the song first and enjoyed everything but the EDM hook, then watched the music video and everything all together made so much sense that I started to appreciate the hook, too.
