Without having watched this beautiful (yes, beautiful) video, I have been using no fewer than 5 of these intervention terms in every single progress note I spend 20 minutes writing. Yet, my supervisor thought it a good idea to say that my progress notes should have even more. Yet, none of their notes for the few clients they see has more than like two. And when I am told that I "meet expectations" when I am trying to and demonstrably "exceed expectations," I never want to finalize a note. I feel like I am being gaslit.
Why would someone think providing any feedback but praise be helpful to someone who has perfectionistic tendencies?

Many supervisors provide an awful experience for others related to documentation and it's really unfortunate.

I have your other intervention hand out with 3 pages of interventions and an area to write out what you did.
This video is more concise. But now I’m questioning everything. So many options. I know it’s personal to type of interventions we do. But I have 60 on my template. They all seem important 😢😮

Oof, 60 is a lot! I would recommend going through your last month or two of notes for clients. I find it hard to believe you used all 60 on a regular basis. Cut out the ones you rarely use, or the ones where there is crossover. For example, in my 3 page list I have both "processed emotions" and "explored emotions" but those are really the same thing. Just choose one.
Lastly, if you really do like having all 60 interventions and use them all, I wonder if you could group some things together and create a second progress note template. For example, if a lot of them are EMDR interventions, maybe you create a specific EMDR progress note template. Hope this helps!

Where can I find the cheat sheet?

  • @CarisaSanchez

Hey I was wondering if you could create a "cheat sheet" or a video about how to fill in the objective section of the note. Sometimes I am at a lost for words and simple duplicate the mental status check box form above (using therapynotes as a note system)

I actually don't use SOAP notes! But the "Objective" section is usually anything observable so think about including 1) Topics discussed and 2) Client presentation. I would just stick with that and move on to the next section :)

If I may suggest, think about factual information do you have in the objective section of the note (results of the PHQ9, like Maelisa suggested what's client presentation, etc.) This is what I learned. Update: However, the fill has a statement that suggest typing "objective info. discussed in session".
