    A lady had Restylane Silk injected 3 days ago for under eye wrinkles, and now has a hard lump under her eye. She wants to know if this is normal, and if she can use hot compresses on the area.
    New York oculoplastic and reconstructive surgeon Amiya Prasad, M.D. reviews her concerns in this video, and explains how hard lumps after filler treatment can be caused by the thickness of the material itself, and stresses the importance of having follow-ups with your doctor:
    0:57 - Dr. Prasad details his background training in cosmetic oculofacial plastic and reconstructive surgery, as well as his experience working with fillers and improving under eye aesthetics in his practice
    1:53 - Why following-up after an injectable procedure is important; and how it takes 2 weeks for filler to stabilize after being injected
    2:39 - How any lumps after filler injection can be a reflection of the viscosity and thickness of the material
    3:26 - How light massaging and warm compress can help, upon the advice of your doctor
    3:57 - How Dr. Prasad’s technique of using blunt cannulas to inject filler helps to minimize bruising
    4:38 - Dr. Prasad discusses another technique called Structural Volumizing, which also makes use of blunt cannulas instead of needles
    5:05 - The importance of having an in-person examination with a doctor; and how Dr. Prasad stresses the importance of maintaining communication and consistent follow-ups with patients

      I had under eye filler almost four years ago. I got the Tyndall effect and have a lump under my right eye now. My dermatologist has been using HIFU and laser toning to deal with it for four months now but I don’t see much difference. Is there anything else I can do?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  2 года назад

      Hi@@michelekoopman6487,
      Sorry to hear about your problem. What was the filler material? If it's a hyaluronic acid filler, it may have encapsulated, and needs dissolving with injectable hyaluronidase. If it's a synthetic permanent or semi-permanent filler, it needs surgical removal. Using thermal devices to remove filler is not an effective strategy. I hope this clarifies things for you.

    Please explain about the Tyndall effect. I have Tyndall effect. I also had a lump. Does Tyndall effect go away after all the filler dissipates? I had some removed with hydrolyaise but Tyndall look is still there.

      Hi Sami The master drawer !
      Sorry to hear that. I normally see filler patients two weeks after the procedure to see how the filler has settled. You should follow up with your doctor who performed the procedure. You can watch this video about how hyaluronic acid under eye fillers like Restylane and Juvederm can be dissolved if there's an issue - ruclips.net/video/WZPJFv3ScsI/видео.html

    Dr Prasad your educational videos are very informative! I really learn a lot from them. I got under eye filler, it’s been 3 weeks now. Small lump and under eye bag formed under my right eye. I tried twice dissolving it 1 week apart, the lump has softened but swelling persists and lumpy feel still persists too!! Can you help in this regard!

      Hi Afsheen Bilal,
      Sorry to hear about your problem. You don't indicate what you had filler treat. It may take more than one treatment to dissolve it. If you aren't already, have the doctor who placed the filler dissolve it as they know the details of your treatment, like where the filler was placed. I hope this helps you.

    I got under eye filler doe two weeks ago and I noticed I still have a little lump under my left eye, it doesn’t hurt when I touch it and I can’t feel it when I blink but it is visible, will it do away with time? I also noticed my right eye needs a lot more filler because there is still a prominent trough, I think they may have put the filler underneath the tear trough because that’s where I can feel it.

      Hi breeze aspire,
      What type or brand of filler did you use? Hyaluronic acid fillers like Restylane or Juvederm are safely metabolized with time, or they can be dissolved by the injectable enzyme hyaluronidase. You can refer to these videos about why hyaluronic acid fillers can stay longer than expected, and how they are dissolved:

      Hi Diana T,
      Sorry to hear that, but temporary bruising does happen often with under eye filler treatment. I suggest you see your doctor who did the filler treatment for an assessment, and to help reduce the bruising. I hope this helps you.

    I had fillers put in my high cheeks area near my under eyes 2years ago and It formed a lump . Does getting it dissolved effect the quality of the skin in the area?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  6 лет назад

      Hi riri r,
      Sorry to hear about your problem. Hyaluronidase does not affect skin quality as it is an enzyme specifically meant to dissolve hyaluronic acid, which is the more common softer filler material used nowadays. I hope this clarifies things for you.

      @@AmiyaPrasadMD Thank you Doctor❣️ Would the dissolving agent do anything to our bodies own hyaluronic acid??

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  Год назад

      Hi@@DW...,
      Injectable hyaluronidase is safe to use as it has minimal impact on our body's own hyaluronic acid, which is continuously being replenished. Your body does also make its own hyaluronidase for turnover of its own hyaluronic acid You can refer to these videos about it:

      Yes, it's possible as age-related skin laxity and volume loss can make scars more noticeable. I hope this clarifies things for you.

      @AmiyaPrasadMD  2 года назад

      Hi@@muneezaanwar4820,
      I strongly advise against fat grafting under thin lower eyelid skin because of the high rate of complications like yours, and have published several videos about it. Lumpy grafted fat does need surgical removal, and is a complex procedure. You can refer to this video about it - ruclips.net/video/mNo_EBcDi1k/видео.html

    Hi, I started seeing lumps when I smile under my eyes 2 years after filler. What do I do?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  Год назад

      Hi Angelika M,
      Sorry to hear about your problem. Hyaluronic acid fillers should not last that long, as they should metabolize in about a year. Also, if filler was the cause of the lumps, it should have been noticeable sooner, like soon after treatment. If they have encapsulated and lasted longer, you can see nearby doctor experienced in under eye filler placement about dissolving the filler with hyaluronidase, as well as examining your lower eyelids for any issues that may be causing the lumps. I hope this helps you.

      @@AmiyaPrasadMD Fillers last way longer than we all thought and many problems start showing up years after getting them filled in. They don't have to - but they can. Maybe you're just a very good doctor and problems don't happen a lot to your patients bc of it, and bc of which fillers you use - but many of us who have fillers by other doctors &have this problems. So... no. Fillers can last VERY long & they can migrate. Most of the time the only way to get rid of it is dissolving, which can cause new problems (can, don't have to) Your video is great anyway :) thank you!
