    Hey Will, I’ve been following your videos for a while. I feel like I’m a version of you from a generation later. I currently work as a paramedic full time , and an RN per diem in a small community hospital down on Long Island. I was a volunteer firefighter for nearly 20 years as well, and left a few years ago, got sick of the “volleytics”.
    The newest NREMTP test is a single online adaptive test that covers bls/als in one exam. You’ll need to take the practical exam as well at an onsite location. Certification can then be maintained by cme or by retesting the written portion every cycle.
    NYS also moved to a similar certification that has been adopted statewide, with the exception of NYC. They’re calling it the collaborative protocols. I have not recertified NYS since 2018, they granted extensions during Covid, so I’m due to recertify in 2024. Best of luck!

      I should have added, I’d focus on the ACLS algorithms and rhythm identification. Stroke and sepsis recognition is important also, but focus on immediate treatments and early notification. Don’t get lost in the weeds on hospital based treatments…think immediate care.
      Trauma should focus on immediate life threats as well and aligns with TCCC type of treatments. Stop the bleed, address chest traumas like tension pneumo and sucking chest wounds, recognize head injuries and signs of bleeds.
      Throw in some other conditions like dka/hhnk, respiratory conditions, OB, overdose, anaphylaxis, and some peds like croup. That should cover most of it. Again, focus on recognition and treatment of immediate life threats.
      You’re still in the game working in the ED, it’s not like you left the emergency medicine field all together. Again, best of luck, and enjoy the shit show out in the field!

      @@user-kf4tt6ij9v Thank you very much!! My last recert was 2013 when we still did separate basic and paramedic writtens and praciticals. I suppose in the trauma assessment they will throw some BLS skills like splinting, etc? I go back to the days of MAST trousers, EOAs (prior to ETT), hare traction, etc! Yes the politics havent changed much, just the faces change. At least now I can stay out of it a lot more being the "old guy"!

    Is there any reason you chose paramedic over PHRN?

      Hello! I live in the People's Republic of New York, a state that doesn't recognize RNs, PAs or NPs in the pre-hospital world. There's some interesting pros and cons to that. My FD rescue squad waits for an EMT to arrive in order to roll, despite me sitting in the truck. I usually end up driving!

      @@DrWilliamMartinPhD very interesting! I didn’t realize that it differed from state to state. In, PA, most of the ER nurses I work with go for the PHRN and we see a ton of RNs in EMS
