What probably happened with the gravestone is for one reason or another it was never used because either the family didn’t have the money for whatever reason or something came up and it never was used it’s unlikely that a cemetery would sell a gravestone that was used
Something else is possible here: In some countries, i.e. Germany, a grave will only exist for a certain amount of time, depending on various things like what type of soil is in a graveyards ground and what graveyard it is, this time is often between 30 and 40 years. Once that time comes close to it´s end date relatives get the option to extend what is essentially the lease for that grave, if you choose to keep it you have to pay a fee to keep the grave and whatever timeframe is set on the graveyards starts new. If nobody chooses to keep the grave or no living relatives are found the grave get´s converted back to what can be considered a unused condition and the space can be used for a new grave. With a system like this a used gravestone could find it´s way to a random weirdo, but they are usually destroyed. Not sure if such a system exists in the US though.
