    I chose this video out of a list from my professor to complete a case study for my senior level Social Work class. This was SO sad. As someone who struggles with depression, I know the feeling. And it's even worse when people around us don't understand or realize that it is a physical problem. In our class, we learned that depression can be just as debilitating as RRMS. I have RRMS, and I would definitely compare the feeling of my legs feeling heavy, brain fog, and fatigue with depression. I sometimes can't tell the difference between whether I'm having an MS flair or a depressive episode until I have a defining symptom. For example, I know it's MS if my legs completely lose function, and I know it's depression if I start feeling suicidal. But there's a fine line at times, especially since they're rarely mutually exclusive. I hope this girl is doing okay now!

    i liked the response of the therapist which was very human and not over medicalised which is a common complaint of clients using the CAMHS service for example - One key thing that is misunderstood; There is no evidence to suggest that CBT is any better than any other modality of therapy - The body of evidence is not comparetive - It is rather like Jeremy Clarkson announcing the Vauxhall Corsa as the best small car by testing it against other Vauxhall Corsas - However that is not to say that CBT is not effective because it is, but so are other ways of working such as Person Centred Approach, Psychodynamic, Gestalt, Existential, Transactional Analysis etc...and when comparetive studies are done all modalities tend to be as effective as one another - The key finding of the research meta analysis identifies the therapeutic relationship between client and therapist as the magic ingredient.

I'm trying yo prepare myself for my new job at a private practice and just watching sessions is helping me get back in the groove of therapy (I've taken three months off since my last counseling job).     Right at the start of the video she said 'It's just another thing that's making me stand out for the wrong reasons'. I would have explored this further, such as, what other things does she think are making her stand out for the wrong reasons? Poor kid. I hope she's feeling better now. As for waitressing-that's going to be hard on the first day. Any job you've not done before is going to require learning/making mistakes etc. She's very hard on herself. She feels like a failure. I'm not surprised she'd depressed. Hopefully, the therapy made her see she's not a failure.

    I understand the use of actors but allowing someone to consent to the use of a real session would be very useful. The reason for this is the the person in the session is demonstrating emotional, physical, and cognitive issues that can not be demonstrated by most actors. It is not just linguistic content that needs responding to. I think many therapists are just robotic in their application. Frankly the utility of therapy is questionable if the therapist is just looking to apply a script they have been taught. The smarter the client the less effective a script will be.

      Chris, there are huge privacy issues around what you are suggesting, so it's highly unlikely that any client would agree to such a thing, and it would be considered unethical of the therapist to ask. These are training videos that are used to demonstrate therapy techniques to students. There is no script, the actress is 'playing' the client and the therapist is reacting to what the 'client' says as it's said. No therapist uses a script in a real session, ever.

    Great videos Dr. Johnson, really enjoy the content during my SSW studies. One thing I am curious about is if you offer transcripts or may provide closed captions. I like to read along with transcripts when doing an analysis to really understand the client/counsellor relationship and pick out certain language worth revisiting. All and all, fantastic resources that have helped my learning tremendously. Thank you!

      Hi everyone, to clarify: the client is played by an actor. However, I am a psychologist (not an actor). The dialogue is not scripted and as such the scene represents a natural exchange.

    Can someone explain her hair style? My friend has done her hair like this for years, and even pins it down this way if windy. I never wanted to intrude by asking her if it was a personal problem or injury she was covering. She never wears it up either.

    I feel personally this client who is an actor is experiencing classic symptoms after a break up.... may not neccesarily be real depression but I felt more like anxiety and greif symptoms which she should be told is perfectly normal until she recovers ....

I'm afraid I cannot give a simple answer to your question, as when we developed this character (the client is played by an actor) we did not base it on a diagnostic category/categories. Here in the UK, Clinical Psychologists tend to emphasise the importance of psychological formulation in order to understand problems, rather than diagnostic categories. Indeed, the British Psychological Society has questioned both the utility and validity of diagnostic categories in their response to the DSM 5.       Thanks Judith - I'm using your videos to revise for the formulation exam for my MSc in Mental Health Studies, all your videos have been extremely helpful!
      In the exam we'll watch a video like this and then conduct a systematic patient history, mental state examination, give a differential diagnosis and alternative differential diagnoses. We also have to explain our reason behind the diagnoses so that's why I ask!
      I completely appreciate there's no way you can make an actual diagnosis - the idea of the exam is just to communicate what area of diagnosis the symptoms of the patient can be best described.

      +Judith Johnson The use a clarity of diagnostic criteria is essential when underlying personality disorders are present. In the cases of BPD and Narcissistic personality types, wether diagnosed or not (most are not, but will be detected via many criteria including transferential responses) they require entirely different therapeutic approaches. So diagnosis is critical. Also, diagnosis and formulation are the same thing when it comes to practice - they are the focus and framework of the work. Labeling is offensive to people especially in a post modern world, ha ha, however thawqt does not mean that discrete criteria do not exist, simply because we care not for clear lines of demarcation. On the other hand, this allows loose unfocused and less skilled practitioners to say that 'active listening' is enough to make you a therapist. This problem with favouring non diagnostic approaches is that there is a very fine line between those that do know what they are dealing with but sit on it, and those that do not and hide behind the lack of labelling standpoint as a mask to hide lack of clear structural knowedge. This is the problem with post modernism - everything is not the same - it is merely a liberal statement - which actually renders all others obsolete - which is more totalitarian than liberal ironically enough.

    I appreciate that the biggest benefit for someone who is seeking help is to be able to talk, I find the session to be somewhat reflective? is that the norm? you ask a question and confirm what you've heard? does this type of technique heavily rely on the willingness to talk?

    What I am noticing is the therapist is repeating a lot what the girl is saying and asking her bunch of clarification questions.

      That is on purpose, some validation, some clarification, highlighting things to a client, and checking the therapist’s understanding. Some times doing this leads to more information as the client can go into more detail.

      That's call reflective listening. There is a purpose to that. It lets the client know we are hearing what they are saying and when we repeat it then it opens the dialogue for more explanation but also clarifies what the client said.

    Dosent this hurt the treatment of cbt when people watch this. Now you can go to the session knowing every thing that will happen. You can kinda do this yourself.

      No because it isn't just what you see on the surface. When we do CBT with clients we are more in-depth about the assessment and literally everything in a session. Plus, we have to made decisions on if CBT is even the best intervention to use so you have to look at them as a whole and what they say and go from there.
