That legit happened to me I had killed her but the dragon she summoned killed me right as I got her remembrance somehow I got the achievement on time. Idk if that's just luck or was a bug, either way I'll take it
Watch again: I think her surprise was at the Tree Sentinel's attack passing, and hitting her through, the 'solid' brick walls. And yes, I know that's how it works in this game (walls/doors, etc. block your attacks but most enemy attacks can, and will, hit YOU through them), but if you're not used to this, it can be a shock the first - few - times.
I insists that Boss health bars are bugged. We see 10X videos of bosses surviving with no health and killing the player than all the other Souls games COMBINED. It happens quite a lot!
One of the things the Vitality stat grants you is a (tiny) chance to survive a killing blow with just 1 health left. Normally this hardly ever happens for the player, but since boss Vitality stats are ridiculously high, they survive with a single HP all the time.

I usually have a 40 stack of crystal darts going. If you see a boss with less than a sliver, back away and toss a couple at em, usually kills em off.
